I get "Setup failed, cannot extract biomes. See above for more details." when I select anything from the ComboBox, including Open World Folder...
How to fix?
Yes, I am getting the same response from my copy of the Biome Extractor:

I am using a personal mod, and this is making trying to map it in all its glory a real pain in the behind!
I wanted to create a mod for Minecraft that makes the eyes die of a sugar overdose. I liken it to finding the gayest unicorn in the most fabulous forest, beating it to a bloody, glam pulp, and then using whatever's still wet as my paint. It might take getting a little used to the colors, especially with the ores. Overall, it's been a very trippy trip mining with this mod.
Anyhow, this is a mod that's best used AWAY from epileptics. DO NOT USE IF YOU GET SEIZURES!!!
In order to use this mod, open the RAR file with WinRAR. If you are a Windows 7 user, then you'll want to go to C:\Users\[insert user name here]\AppData\Roaming\.minecraft\bin and open minecraft.jar with WinRAR. Drag the picture files (NOT the folder itself!) from the mod to its corresponding place in the minecraft.jar file and hit OK. Once you're done with replacing the pictures, close out of everything, start Minecraft, and VOILA! Enjoy!
