If your going to use the 'latest' version of NEI then use the 'latest' version of Chicken Code Core. If not, then use BOTH the 'recommended' versions of each. One or the other, never mix them like that.
You're suppose to use the 'universal' downloads. The only reason your not getting the left side is because it is locked in recipe mode. Going to your inv. screen in-game and clicking the options button down in the corner. From there hit the 'inventory' button then that 'global' button in the upper right corner, then just unhighlight/un-check the "W" you see by recipe/cheat mode (just right click it) And all your left side will return.

in-game go to your inventory screen and hit the options button down in the corner. Hit the 'inventory' button and then that 'global' button you'll see in the upper right. Now un-check/un-highlight that "W" you see by the cheat/recipe mode, a simple right click on it will do.
Your a Mac user running on java 6, I'm surprised your game even loads (unless something changed I'm unaware of) I would try using the latest Java 7 and see if it persists.
