minecraft is the kind of game that updates over a period of time, every so often they come out with a new release and new features. mojang have said that they don't plan on stopping development on minecraft until the community dies, so mod makers/forge/any other modLoader just has to keep up. also just imagine if minecraft stayed as it was when it was released at 1.0, would you play it right now with all of the other games that have come out since then and they way games have changed? if no then that's why they are updating, if yes then that's your opinion and not many people share it and you can still play 1.0 and i'm not sure if mods have been made for it.
its rare for that to happen now. the algorithm looks for solid unobstructed ground to place a portal on when you go into the nether. this could happen if there is an island of nether rack in the middle of a lava lake near the coordinates. in that case just build with cobble to the nearest land and your good. make sure to make it out of cobble or stone because ghasts cant brake it, also out at least 1-high walls on the edge so you cant be blasted off.
ive seen this dungeon before. it was in a mod pack. the only problem is it was 3 years ago and i don't remember witch one. this is definitely modded dungeon not added by any of the ones you listed though. are you running a mod pack and those are the ones that you added?
there was a mod in that pack that added traps (i can't remember again) and it would spawn structures that had the traps in it. they would look similar to this however they weren't as massive and had more obvious modded blocks in them.
i think it depends n the kind of server. for example if it were a survival server i would want to choose my own texture pack to fit y mood, if it were a pvp i think 3D would look much better. but as a server owner you have to consider not everyone can handle loading recourse pack bc their computers are not up to it. there are a lot of mini game servers that the average mine craft players can't play bc of their over use of 3d texture pack and having to switch between them for multiple different games/areas. so if you putting a pack on your server try to remember this.
