Your IP adress may change, but it doesn't have anything to do with the port. For open a port, you have to use your LAN IP, not the WAN one. LAN IP is this internal IP, something like 192.168.x.x. Remembering that you need to open port 25565 both TCP and UDP. Review all your config, if possible post here how your port config looks like and i'll try to help ya! Remember to tell your modem model too.

If you're running Windows, Start->All Programs->Acessories->System Tools->Restore Point.
Anyway, this is not mandatory, it's just good to have it if something goes wrong.
