Texture packs have a strict folder structure. If you open up the .zip file you should see the terrain.png. You should NOT see it inside a folder that is inside the zip.Quote from diamondking

Correct: myPack.zip/terrain.png
Incorrect: myPack.zip/myPack/terrain.png
This is the most common mistake new texture pack creators make. You basically have to mimic the folder structure of minecraft.jar when setting up your texture pack. If it's not in a folder in minecraft.jar, it shouldn't be in one within your pack. The opposite also holds true, for example your paintings will be in "myPack.zip/art/kz.png". See?
If you're absolutely sure that this is correct, try figuring out what program you're using and check your compression settings. I know a few people have complained that the default on a mac compresses it beyond the point where Minecraft can read it.

Kidding aside, here are a few things to check:
1) Is the terrain.png in the root of the .zip file? It should be.
2) Did you move the .zip file into the /texturepacks/ folder of /.minecraft/? You should have.
3) Did you remember to actually select your pack from the Texture Packs option in Minecraft? It's right below the Multiplayer button on the main menu.
If none of these obvious things is your problem, then help us to help you. What program are you using to make your textures? What program are you using to compress you .zip file? Any information you can render will be helpful to us.
