This has probably been suggested before, but why not simply have the account name be separate from the player name like in pretty much all other multiplayer games from the past decade?
The account name could still be used for white / black lists etc. but never displayed in-game.
That's the way it should be, but now it's unlikely to change.

The reason I didn't recommend against it is because it may have been a configuration error or some odd conflict that caused the scenario.

Those you mentioned seem to be the most widely used ones at the moment.

I'd like for the atmospheric fog to have a slider. Sometimes it's just a litte too much

It should be possible, depending on which permission system you are using and what chat plugin.

If you could provide the make and model of your router, it would be much easier to help you.
What you need to know is the LAN IP address of the PC hosting what you want forwarded.
So if the server in question is using the address then that's what you enter in your router configuration. The standard port that Minecraft uses is 25565 so that is probably the port you wish to forward.

bPermissions, PermissionsEx, and GroupManager are the ones most commonly used.
If you want the ranks to show up in chat, you'll also need a plugin for that.

It's a bit hard to tell since you aren't providing much information about what your players already have access to.

A high end gaming PC usually focuses on the GPU which doesn't help server performance at all.

Most people running a server with 100 people on wouldn't be running Windows or OS X because of their memory consumption.
Running a Minecraft server with that many players will consume a lot of memory. I'm not going to state any numbers because that would depend on your configuration too.
If you are serious about your server, consider some distribution of Linux or FreeBSD.
