This is fairly basic, this happens often on SMP and sometimes on SSP, your computer just "forgot" to load a chunk and if you restart the map it should work.
That's a glitch. The game crashes when a piston removes its own power source. Have you tried restarting the server? (I dont know what you mean with restarting the map)
I think you can open the map with MCEdit, where you can remove the piston and the lever. That should work.
I have tried restarting it, the map just reloads the broken piston and crashes the server again, i'll try using MCEdit to remove the piston good idea
EDIT I tried removing it (I just downloaded mcedit for this reason so I might have failed at it) but the error still comes up and I can't connect to the server, Is there a specific reason why I can't delete the piston? I tried deleting the entity and the blocks

New bug i was trying to make a sort of fancy door for my house while i was messing around with the new pistons, i had the piston right below the lever and when i flipped the lever it pushed the lever off the wall, but the lever couldnt be picked up and my server crashed shortly afterwards
Ok so a friend and I are playing on my server which I got up and running a week or so ago. When the new piston update came out we immediately tried our best to integrate them into out houses and mines. Then a glitch occured where I put a lever on the block above a piston then when i switched the lever on, the piston pushed it off the wall. But after that the piston couldnt be picked up and the piston disapeared, but the black - lined box still appeared around it. Shortly after the server crashed and the log was spammed with this

at java.util.HashMap.get(Unknown Source)
at mz.a(SourceFile:136)
at mz.e(SourceFile:104)
The two final ones alternating at least 100 times, and now the server won't connect to anyone, including me and on restarting the map replays the error making it do that again. Anyone know the cause of this and/or know a way to fix it?
