How to get a commandblock with NBT: right click+ctrl on a command block
You can also do it with /blockdata
And if i'm right also with

step 1: open up world
step 2: get a command block
step 3; start doing stupid things like getting some weird swimboots wich give you a butload of particles when in water
stupid ideas is the key, try messing with scoreboards, dropping items, /entitydata and /blockdata.
learning is way easier that way, btw: youtube might also help

I think the enchanted books needs the StoredEnchantments tag
Just put down a chest, put an enchnted book inside it, type /blockdata x y z
And you'll see that tag and you can add it to the command and modify it, keep in mind brackets do exist

But yes, reply above, no quotation in those entity specifiers

In a map i'm currently working on i used a "fairy ring mechanic"
I used armor stands wich get a tag and a name change when the player unlocks them by standing near them, pretty tricky but doable concept
But i changed the tag gives them a spark and the name makes them say something in chat to teleport to that location if you are near a fairy ring, if ylu want to know all command i'll get up my world and give you all the commands

That are for me the main aspects, but people also prefer command block maps, just where the command blocks take over...
(Commandblocks are very easy to learn, youtube is your hero if you need the help)

i do also have a simple solution for you, make a little structure that isn't refindable/recreatable in survival (like a bedrock tower wich is 5 tall)
and make it a portal by always having an execute command active ( /execute @p ~ ~ ~ detect ~ ~-1 ~ minecraft:bedrock 1 /execute @p ~ ~ ~ detect ~ ~-2 ~ minecraft:bedrock 1 /execute @p ~ ~ ~ detect ~ ~-3 ~ minecraft:bedrock 1 /execute @p ~ ~ ~ detect ~ ~-4 ~ minecraft:bedrock 1 /execute @p ~ ~ ~ detect ~ ~-5 ~ minecraft:bedrock 1) /tp @p 0 0 0)
but do this in the end or the nether in an empty space far from anywhere so they will never find the dungeon while just exploring
then pop down an armorstand in the area the player gets teleported too ( /give @p minecraft:armor_stand 1 0 {EntityTag:{NoGravity:1,Invisible:1,Invulnerable:1,Tags:["Dungeon"]}} )
/execute @e[tag=Dungeon] ~ ~ ~ /gamemode 2 @p[m=0,r=5]
wich leads into a conditional chain
/give @p[m=2,r=5] minecraft:shears 1 0 {CanDestroy:["minecraft:wool"]}
this should be 99% safe for any block breaking

And a start button with different command blocks wich all have a slightly different command
