I'd suggest a silverfish sort of drags the nametag. The nametag would basically stand upright like a sign, and it would be its own entity that's "tethered" to the mob. Most of the time it drags, but if the mob turns to face the nametag, then it would be in front.
For bigger mobs, I'd suggest a sign hanging at chest level, but I'm also fond of the idea of the tag flapping around somewhat independently.
This would render name tags useless unless they still maintained their current system as well. So this seems to me just unnecessary.

It could behave like instant damage and instant healing. Not with a timer, like the other effects. I actually would like this; higher levels could have longer fire durations. It would allow for remote setting and removal of fire from players; certainly useful for adventure maps. So I do support the potion effect if nothing else.
I actually really like this, however I believe it should be crafted with a cobweb as opposed to string, for balancing purposes. Upon breaking it, it should work the same way regular cobwebs do now, only dropping itself when broken with a silk touch tool.

The only possible reason would be the difficulty in programing it... which I believe wouldn't be that high.
I'm not very knowledgeable on this subject, so I may be wrong however.

That's the kind of stuff you can do with this; except it won't be limited to set coordinates. Like I said, this opens up a lot more possibilities.
