Try allocating more ram which can be done with the start.bat script. Although do not allocate too much considering your computer only has 5GB of ram. Which is strange I've never seen that before.
To add a command to a user it would be "/pex user <username> add <Permission node>" For a group it would be "/pex group <group> add <permission node>" I hope I'm understanding your issue correctly if not I'll check up on this topic again and see what I can do to help.
If I remember, those are the "100% free" servers which are likely home hosted servers by someone in their own home on an business line. You do know they are likely trolling the living heck out of you by playing on the same PC the server is running on? Hence how they have ::1 (127.0.0.1) as their IP. It is not possible to my knowledge to spoof the packet header IP to such for minecraft, but never saying isn't possible either.
I'd have to agree with this post. As for the hack, to my knowledge no such hack exists. Possibly he had access to console? Or even admin commands?

Hello, I myself have little experience with lillypad but I'm assuming it's similar to bungee. So here's some things I thought of.
When IP-Banning someone With Bungee the Command will ban the Hub Proxy. Are you making sure Staff member are not using any IPBan commands? Assuming A command to IPban is available in one of those plugins.
Does it specifically say Banned? Maybe the server is refusing the connection to the player.
Do you have staff members trolling? Check your Ban list and make sure none of the players are being removed and added.
If any of these didn't help Add my on Skype, mickeyjay88. (The one who lives in MickeyVille, Canada) Just make sure you put your Forum name in the friend add MSg.
