Which OS and Java are you using while this problem occurs? Are you using a Mac? I would like to know which configuration this problem happens with. But you say that the 2.9.0 nightly build works ok? I might move to that at some point then, I just don't want to move to an older version if the current recommended build works perfectly for me.
My system is Windows XP SP3 running on an Intel Atom, with Java 7u9 (or possibly 7u8 at the time). PS2 keyboard, Bluetooth mouse. Weak CPU and graphics, so I tend to tweak the system regularly for speed.
Crouch broke when standing on full blocks. I think the sliding off of half-blocks is intended. Also had problems swimming. As soon as I clicked on something, I sank like a stone
Had no problems with 2.8.4, but tons of problems with simultaneous inputs with 2.8.5. I'm currently using lwjgl 2.9.0 build 1903, found here. No problems, or none that I can blame on lwjgl, anyway
I wouldn't say that 2.8.4 is old, it's just from May 2012. I think standard Minecraft is still on 2.4.2, from April 2010, so just about any upgrade will be an improvement.
They don't. They reset during the upgrade process, which, among other things, also changes the minecraft.jar file. Most probably a bug.
It also resets if the game find any unrecognized stats or achievements. If you have created something that depends on a mod, and then removes that mod and restarts the game, all stats and achievements will be reset. Which is just bad programming.
I'm not going to watch an 8+ minutes crappy home-made video just to speed up diamond farming for the price of a load of iron I don't have.
I'm a speed reader. I can read (and comprehend) ten times faster than being spoon-fed by YouTube. I know that a picture is worth a thousand words, but this technique is like 5 lines of text.

Specifically, when placing blocks while crouching, the crouch broke every time I placed a block, and I had to re-apply the crouch over and over.

But stay away from 2.8.5, it's buggy. Go with 2.8.4 or the 2.9.0 nightly.

No, they're not. Achievements are stored in .minecraft\stats\stats_(user)_unsent.dat

Same problem here, except it works when I go back to OptiFine HD_U D3. D4 and later seem to disable EnchantView in some way.

Works well in Creative Mode, Minecraft 1.4.5. I wouldn't be surprised if the timings were thrown off by the upcoming Redstone Update (1.5).
