Upon further investigation and visiting the Xbox forums, it appears that mobs in the Nether are spawning more frequently, to the point where they are hitting the cap constantly! Blaze spawners are still working BUT the cap on the world is being kept constantly, resulting in the spawner being unable to spawn new Blazes!
