The modern liturgical Latin of the Vatican which would be "XVI Junii, MMXII" or the Classical Latin way which would be like this "ANTE DIEM XVII KALENDAS IVNIAS MMXII"

Shpaget is right. Dedicavit is the perfect past tense form it's seems I missed that. I believe you need to change it to "dedicari" which is the present passive.
