At the nexus of my cave system I have a void on one side and a water fall on another. Both are walled off. The waterfall will at times collect bad guys at which time I break a single block, clear them out, and a current takes them into a safe pool on my side of the wall. From time to time they are holding rotten flesh but tonight one was holding two slime balls.
Do these items indicate a slime died somewhere and the zombie picked up slime balls or was this a random item spawn that was created when the zombie spawned? The biome is plains btw, but I am down at y=16ish
Most probably a slime that got killed by lava or a drop... Weird though that it was holding two slimeballs. As far as i know, zombies can only hold one item at the time.

Quite possibly I might have killed two zombies each holding one, and ended up with two in my item clean up.
What I am more curious about is how did slimes spawn in a non-jungle biomes, F3 says it is still plains just deep in the ground...
Do biomes change when you change elevations? Could I have a pocket of 'jungle biome under a plain biome and a plain biome in a cave?
Slimes will naturally spawn on the surface (or there abouts) in a Swamp biome.
In all biomes each chunk has a 1 in 10 chance of being a "Slime Chunk" in which slimes will spawn from y=1 to y=40.
Some part of your area is in a Slime Chunk and is lower than y-40, that's why you have slime balls.
slimes will not drown in one block of water, yes? Will the smallest sized slimes die in one block?
That seems to be the explanation that fits the best but after several days of mining and being in the main room, and cleaning out the waterfall of spawned monsters several times, I have never seen slimes or slime balls.
I think the slime balls were probably picked up by a wandering zombie, before the zombie fell into the water stream. As of 1.8, slimes will now swim like regular mobs (previously, they would just sit on the bottom and drown). The slime ball was probably dropped by a small slime that fell off a 4 high (or more) ledge.
