Unless you have a thick number of players and if it's a vanilla server, you should try to lower maximum ram, say to 4GB (I also recommend for “production” servers to always configure same starting and maximum ram).
This said, usually problems with tps (tick per second) and tick behind messages are a sign of CPU overload. This may be a busy CPU from other processes (Java server is very demanding!) or some game condition that causes very high CPU load, like a lot of entities from farms. Usually mobs, animals or villagers - expecially large concentration of villagers - in always-loaded chunks can bring a server CPU to 100% whichever CPU you have.
I'm assuming vanilla here, but with mods both memory and cpu usage go ramping up more easily.
Do you have any idea on how to lower mob spawns then? And yes this is vanilla, the original world was a bukkit server but it was transferred to vanilla.

Not sure if it will work for you first off
1. java -Xmx12288M -Xms4096M -jar server.jar nogui change the second XMS to the same number
2. Close the server sometimes just to let it rest for a second then reopen
3. If no one comes on that server does say it is behind but when someone joins it's fine
