NOTE: I used to have this GPU installed a few months ago and everything worked perfectly and my games were running at smooth FPS. However I uninstalled the GPU and removed it from the PC as I was planning to upgrade, but never got round to it.
I recently restored my PC to factory settings and installed a fresh windows 10 and decided to reinstall the 960 again. After that I used DDU to uninstall my graphic drivers (not sure if this was necessary after a system restore) , and then reinstalled my gtx 960 along with the nvidia software and latest graphic drivers.
However, when playing Minecraft I have extremely poor fps averaging 5-10fps even using Optifine and nvidia optimised settings on Minecraft. The display quality is also very poor (the black shadows and white/coloured dots etc.) as you can see in these pictures:
Surely this isn't right? The fps/quality is that poor the game is unplayable, but I never had these issues when I had the GPU installed a few months ago? What should I do?
Thank you very much for reading and any help is massively appreciated!!
Say something silly, Laugh 'til it hurts, Take a risk, Sing out loud, Rock the boat, Shake things up, Flirt with disaster, Buy something frivolous, Color outside the lines, Cause a scene, Order dessert, Make waves, Get carried away, Have a great day!
I just followed these steps and the issue still persists. When I press F3 in-game it still says "VMware, Inc." next to the display section like webrosc pointed out.

Ahh I see. Sorry I'm a bit of a noob, whats a dxdiag log and how would I do that?
Thank you for the reply!
Read webrosc's signature. You can also just Google.
Thank you. I have pasted a dxdiag below:

Just noticed your signature saying to quote you...
I have pasted a dxdiag above
Thank you
Update your NVIDIA and AMD drivers.
I have already updated my nvidia graphic drivers to the latest ones. Why would I need to update my AMD drivers?
