i want to upload the dxdiag log along with the crash reports from the %appdata% folder but can't see where to include it
Reinstall your Intel and NVIDIA drivers from scratch.
Get your driver from Intel and NVIDIA sites.

When you reinstalled Windows 10, did you use the original ISO or a newly downloaded one? Reinstalling windows could have reset your driver version to an incompatible state. What process did you use to reinstall Win 10?
Also, it looks like you are using two monitors (one from your laptop and and a connected monitor). You also have the integrated graphics (HD Graphics 530) and a GeForce GTX 960M. You will need to check both driver versions are updated. Are you able to see what graphics card is being used in the debug screen?

Post a new DxDiag log.
Mate this is community support, people here aren't getting paid to help you. But mostly if nobody answers it's because they don't have a solution.

Solved issue on mine and might work for you. Feel free to try but there's not guarantee.
Download Rufus and reinstall older version of windows 10 (hope you have your product key) like 1809.
The install/update drivers, antivirus, games, windows updates, etc.
