I've been playing on a friends Realm for a while now and ended up building 4 0-tick bamboo farms, I had all the bamboo get droppered into a water stream and led to a single hopper that transferred all the bamboo into the storage system. The hopper was unable to keep up and items would pile up, and I accidentally left them running while I worked on other stuff quite a ways from the area. Upon returning my game freezes and I time out from the server before it rolls back.
If someone who was opped was able to run /kill @e[type=item] the instant those items became loaded they might be able to kill them before timing out. Otherwise you'd need to dowload the world, use something like NBT explore to edit the world file, and delete the items from the chunk they're in.

Any suggestions for dealing with this?

Or just restore the world from backup as realms creates backups automagically.
do you think a command block with a red stone clock set for like every tick would be able to work?
If you're gonna do that use a repeating command block
