I built an iron golem farm, 20 blocks above ground level, with 2 cells of villagers, 3 in each cell, on each side of the spawning platform. Each cell contains 3 villagers, 3 workstations, 3 beds and it well lit and covered. Any time I go away from the area, I come back to find one of the cells empty of villagers and no clue as to where they went or how. And it only happens to this 1 cell side. The other cell never has any problems with missing villagers. No mobs can get to them, they can't drown, they can't get stuck in a wall and suffocate, they bob in a half block of water and, again, nothing ever happens the identical cell across the way. It's all done in survival and it's getting really annoying to have to build new rails and paths to get new villagers into the cell to make the farm work.
The villagers are most likely despawning. I would suggest using nametags and naming each individual villager. If you don't have enough nametags you can get them by fishing using Luck of the Sea III.
Each cell is completely covered, and I have tried to nametag them all before, too. I have 100s of nametags, no biggie there. And as for witches, it's covered and only happens to the 1 cell, never the other. And they are blocked in, and no witches would be able to escape, even if it did happen. This is frustrating me to no end. And it happens whenever I travel away from the chunk, even with no storms, etc.

Villagers cannot despawn. But they can die or turn into witches by thunderstorms.
