Let me explain. I log in. I play for 5 minutes. I'm fun and games on my SSP world (on creative this doesnt happen, neither on multiplayer). When too many chunks are loaded, this happens:
when I click on "save and quit to title", nothing. I double click this button and appears the Windows 'loading' icon on the mouse cursor. Then the screen gets white and Minecraft closes, leaving only the launcher, saying "Can't keep up! Did the system time change, or is the server overloaded?"
What to do?

Do you play with mods? Even if something small like Optifine or MCPacther.
What version Java do you have installed? Go to "Start", click Run or Search, type "mcd" and hit enter. In the command console type "java -version" Type out your results to us.
What video card is in your system? a couple ways to get there, but fi this is your computer, you should know.
Just to be clear, this is on Single-player only that the problem happens?
Latest version of Java (I thought that this would solve my huge need of RAM, but didnt)
Mods (Optifine, Xaero's Minimap, Amberoguia, Tree Capitator, Swords++, Zombie Steak, Backpacks), HOWever, this happens sometimes when i play without mods, too.

As I know, only happens on SSP, but idk if its the world (because in a superflat world this doesnt happen) or if its the gamemode
On Multiplayer (factions look more like a SSP), this doesnt happen (well, once I went from x=0, z=0 to x=12000 z=6000 and nothing happened, at least to me)

I tried to install something to clean RAM. I tried to finish useless processes, like Hamachi and PS3 controller. I tried to allocate more RAM to Minecraft (right now, my Java arguments are: -Xmx1200M --> thats for 2GB of RAM my pc have)
I turn it off every night, better, every time im not using it, i turn it off.
this is getting annoying, because every 5 minutes (or every portal I enter, or every 250 blocks I travel) I have to log out

Your CPU or RAM is being overloaded. Avoid running multiple programs. Your PC build doesn't seem to be ideal for gaming.
Yes, it can run games, but it isn't built for that purpose. Your graphics card is capable of running all the GPU calculations required for your visual mods, but somewhere in there, your CPU is bottlenecking.
In your server tests, is it running on the same machine?
Another possibility is one of your mods is bottle necking, like if it is a poorly written mod, and it does all of its render calculations on the CPU, that'll choke your CPU, and halt your game.
But since you say this happens in vanilla as well, one thing you might want to try is to monitor your RAM and CPU usage in your task manager. See what makes it climb, and like I mentioned earlier, close any unneeded software.
Also, check that you don't have to Java processes running when doing SSP. I know the launcher has issues sometimes and fails to properly close itself, and runs in the background. If you see 2 processes, close the one with lower RAM usage.
Minecraft is resource intensive. More mods means the problem gets worse. Make sure any background programs are closed. Anti-virus may also need to be disabled, just for enough time to test.
It might also help to provide your logs (in spoiler tags) to see if something there is causing the problem. But I am quite confident int his being a system issue.
Also, RAM cleaners are a myth. System optimization tools are a myth. You might want to spend time in uninstalling programs you do not recognize.
I only have this pc, so everything is tested here.

Sometimes the launcher glitches and doesn't listen to what option you gave it. But you are saying your GPU drivers were crashing. Are you running the latest drivers? If your drivers are beta, try rolling back to previous drivers.
As far as that goes, I can't really think of what else it would be, especially if this affects you when you have the issue in vanilla as well.
If your memory footprint from your anti-virus isn't significant, I can't see a reason to close that, unless it has real-time monitoring which will use up your CPU resources. The other trick you can try is to set your process priority.
After your game is launched, open your task manager (either right click task bar, and start task manager, or hit control shift escape). Then right click on the java process (Minecraft), and select "set process priority". There are various options in the menu list, so try incrementing the value to try to fix it. Do NOT select "real time" in case if it crashes your PC (it can cause your game process to claim all available resources to run at full speed, including resources which should be available to Windows processes).
I don't know if process priority settings are permanent, so if they aren't, you can always do it again as needed. Also note, Normal is the default settings for every process. You can further improve this by turning down the priority of processes, such as your anti-virus, which doesn't need to be calculating as often as your game.

Two things cause this issue: Driver instability, which requires a reinstall of the drivers or general system instability, bottlenecks, used resources...
The CPU you use is not really good for gaming. Maybe older games, but not newer titles. If you continue to have issues, just play Minecraft without mods. Save up for a more up to date system.

I drag my system through the mud, a lot. I cause issues. I know I cause them. That is how I use my system. once every 6-8 months, depending on what I do to it, I reinstall Windows. Of course, that is a solution for me. Minecraft launcher provides enough Java arguments for vanilla. So, to try and get around that, you can modify it if you need to. I have found that MultiMC (an alternative launcher) provides me enough leverage on my older computer to modify the settings and for some reason runs better than if I launched through the Mojang launcher. Might be an option to see if you can lessen the issue. But, you are still going to have a problem when using the system you are currently on.
Or refresh. Reinstall Windows and start over. Just follow steps to backup what you want to keep.
hey, now, for some reason, changing the priority worked. I went to Nether 3 times and it saved and quitted normally. hey, now, nothing runs along with Minecraft

but 1 thing we didnt talk about: i can use Ubuntu!
my other horrible pc has ubuntu and it runs nice, and i saw somewhere that Ubuntu needs only 512MB of RAM, while windows needs 1GB.
this may be a good thing to quote, as i cant allocate more memory than 1200 MB

That IS a possibility. If you feel good enough for it, use Ubuntu. In fact, you might get a little more performance by putting Ubuntu on your current machine. Just try to get the latest version (can't be bothered to look for the version number) as the desktop display server has been improved plugging tons of open memory leaks further improving it's performance. I would switch to a Linux flavor myself, but I am still playing lots of games that I just have not been able to get running in WINE. So for now, I'm still sticking to Windows. Might be for quite some time.
If possible, though, get another 1 GB of RAM on your current machine if possible. It might help. Just don't pin all your hopes, the CPU may still be a bit of a bottleneck. even with Ubuntu.
I think ill make dual-boot. This way i can play on both OSs and i wont need antivirus, coz, u know, ubuntu = no viruses

It's still a dangerous mindset to have. Best to get an AV that reads all incoming files to your system so as to not share thos einfections that would affect other users on Windows.
The command line is used even in Windows, and Macs. You'll find Linux is very similar to Macs int hat respect, as Mac was originally sourced from BSD way back when. A lot of design choices are still around. Macs are seeing a rise in infections as they become increasingly popular. So are Linux systems. Always be cautious and you'll be fine.

System time cannot keep up if your CPU, RAM and GPU are bogged down.

Sounds off the wall, but how often do you restart (not go to sleep or hibernate) your computer?

Be careful. You can still spread virus/malware to other people. And that is a myth. Any electronic device can suffer from them.

It's still a dangerous mindset to have. Best to get an AV that reads all incoming files to your system so as to not share thos einfections that would affect other users on Windows.
The command line is used even in Windows, and Macs. You'll find Linux is very similar to Macs int hat respect, as Mac was originally sourced from BSD way back when. A lot of design choices are still around. Macs are seeing a rise in infections as they become increasingly popular. So are Linux systems. Always be cautious and you'll be fine.
