Hi, before I start, let me mention that this has been an ongoing issue that has been going on for months. Going into more detail, the issue itself is that java/minecraft constantly crashes when I log onto a server. I'm not completely sure why but even when creating my own server with no plugins whatsoever so I could play some survival with a friend, my minecraft crashed after a few minutes. The duration I can stay on a server without crashing varies, sometimes it's a few seconds or minutes but other times I crash almost immediately.
EDIT: I'm using the latest 64-Bit version of Java.
Here's my system specs:
Processor: Intel(R) Core(TM) i3-3217U CPU @ 1.80GHz
Installed Memory (RAM): 4.00 GB
System Type: 64-bit Operating System, x64-based processor
Graphics Card: Nvidia Geforce GT 630M
Here's the crash report:

Bump?
Try turning off VBOs in the video options.
Thanks for help but I just removed everything to do with minecraft (.minecraft folder) from my comp and the problem seems to have resolved itself.
VBOs are off by default, so you may have just done a fairly expensive reset to default options.
