The developer of Optifine is denying it's due to OF, but I tested and it only occurs when OptiFine is running, since the 1.4 versions were released. There appears to be something unique to MC 1.4 that is not playing nice with OptiFine.
Since the OF developer can't seem to track down the problem, I think we have to hope it goes away when they update to 1.5.
I have kept digging, and it appears that as part of 1.3 (yes, that's one point three), Mojang fixed a bug related to "signs not loading on the client side." It seems that whatever they did made things worse, not better, at least when running OptiFine under 1.4.
Whatever it was, it didn't cause a problem with Optifine under 1.3, and only became evident under 1.4.

I think its REI's Minimap causing it, that seems to be another common mod between us... but I don't have any signs lying around, I don't think... so.... no clue what its talking about?

I'm so glad to have found this thread that I had to join the forum so that I could share my experience with this.
I also have this issue, and it's not particularly random. Whenever I cross over certain spots on my map I get a message saying that it is unable to locate some signs, and gives me the coordinates. The only mod I have installed is OptiFine, though I do open Minecraft using Magic Launcher as well. I was pretty certain that OptiFine was causing the problem, at least for me, because the issue first arose after downloading an update to OptiFine. But before I posted here I wanted to be sure, so I ran Minecraft without OptiFine and Magic Launcher. Sure enough, no issue. I then opened Minecraft through Magic Launcher with OptiFine disabled, and once again there was no issue. Finally I re-enabled OptiFine and, lo and behold, the problem came back. I don't have Rei's Minimap (or know what it is, for that matter), so I can't test whether it would cause the same problem or not.
I wish I could remember exactly which OptiFine update it was. Fortunately, I'm new to OptiFine (and to Minecraft for that matter) so there aren't too many possibilities. I feel that it must be the 1.4.5 D4 or D5 update.
Do the developers read these threads? I hope so. This bug doesn't break the game, but it's annoying.

Apparently the problem existed for some people prior to MC 1.3. At that point Mojang did some coding which appeared to work. But it resulted in the problem re-emerging under two conditions: a Bukkit issue for multiplayer server games, and an OptiFine problem for single players.
The devloper of Optifine has remained almost totally silent on the problem, despite lots of people posting. He made one statement which said it had nothing to do with Optifine, and pointed to the Google searches about Bukkit problems. When folks came back and reiterated that they were experiencing the problem ONLY with Optifine, and in single player, he went silent again. It's clear he has no idea how to fix this, and isn't talking about it.
The problem started with OptiFine 1.4.3. I believe if you roll back optifine, it goes away. Which is further proof that it's an OptiFine problem.
Again: this also occurs with Bukkit for multiplayer.
The thing I don't understand is why it doesn't seem to be a problem for everyone who uses it. I just read the last week's worth of posts on the OptiFine thread and only a couple people mention it.
But whatever... I'm content to wait and see if things get patched in future MC and OF updates. Obviously they are aware of the issue, even if they don't yet know what to do about it. I admit, though, that it bothers me that it's not on the "to do" list...
There have been quite a few people who have posted it on the OptiFine forum. The problem is that it's one huge thread now, almost 1400 pages long, and posts get lost quick.
Usually the developer is a cool guy, but on this one he's stuck his head in the sand and is just refusing to acknowledge there's a problem. Continuing to point to lame Google searches for multiplayer issues, when single player folks are insisting it's OptiFine, means he will never fix it. Frankly I think this problem is over his head and he won't admit it. Kinda sucks, especially when you have a map like mine that is dependent on signage.
Which may be why some people experience it and others don't: the problem may only surface when you have more than X number of signs, or X number of populated chunks.
If Forge would also support animated textures CTM and that stuff we could get rid of optifine as nobody who uses Forge would need it anymore ^^ but I guess that isn't possible
Well forge support HD packs as far as I know but as far as I also know it does NOT support CTM and animated textures - since the TP I currently use uses both I'm dependent on OptiFine since Forge doesn't like MC-Patcher
Then you're not using Optifine for its intended, primary purpose, which is to enable MC to run on older machines, or to improve performance. As a result, suggesting other people "don't need it anymore" is not quite accurate.
I have the same problem with the 'disappearing sign text' stuff but when I stay near the disappeared sign and return to the main menu then come back to the map, the disappeared text came back! Works for me.
In the 1.4 update Mojang edited a bit of its rendering code. Maps created before 1.4 have a chance of creating a bug with signs in which a chunk is corrupted and then forced. In doing so the chunk appears sunken per your client. Normally you wouldn't notice the difference as the chunk can appear hardly changed but the sign is not aligned as it should which in turn makes it so the text and information for said sign cannot be displayed.
This again is per your client. And can happen with ANY mod that accesses the class files for map rendering(I believe but have yet to find which files specifically.)

How to remedy? Well From what I've found is by left clicking in the bugged chunk the true items are then brought back. Once you connect 1 side of the chunk to another you can then relog and that specific sign is temporarily fixed. (Then break the sign and replace it.)
Edit: This is what happens to me, I have only been able to recreate this specific problem dealing with the 'unable to locate sign[location]' . I urge you to try and break any signs and replace them if they were placed before 1.4.
Most of the time, for me, nothing much happens. But when I explore, I get this message as well. However, it seems to only be in certain spots. If I leave the spot, then come back, I get the message again.
Not sure if this is related, but sometimes my item frames in my house will disappear as well upon loading the world, or just leaving to go exploring, and coming back.
After removing Optifine earlier, I loaded up my single player world, and this error still pops up, not being able to locate signs. I have a few other mods, but the only other ones I have that everyone else here seems to be using, are Forge, and Rei's minimap. When I play with a client that doesn't have either of these mods, the error never appears, though some signs still pop up blank. Perhaps this could narrow it down?

For more info this in on SSP, I have only optifine and rei's minimap as mods, and my location was -504,10,-209.

I am not using Rei's Minimap.
