In Java Edition cannot get smooth stone to use any texture other than the default texture. The default texture is listed as "smooth_stone.png" yet it refuses to use it.
In the bedrock edition I have the same problem, yet it uses the "stone_slab_top.png" The java edition does not do this.
The texture I am using is the same as the aforementioned "stone_slab_top.png" (copied and renamed) in my texture pack for java and stone slabs use it fine, yet smooth stone does not.
I'm wondering if this is a known issue or if there is a fix at all. Thank you.

Which version of Minecraft are you working with?
What is the pack_format in your pack.mcmeta set to?
Do you have a model in your pack for stone slabs? (If so, either delete it or make sure it's pointing to the correct texture)
Do you have any mods installed, including Optifine?
Does this only fail with your pack, or do other packs work? (Use one that's for the MC version you're using. Lithos is up to 1.16 if you need a suggestion.)
Will you please post a session log from a session in which you've attempted to apply your pack to said stone texture?
You haven't given enough information for us to diagnose the problem. You're on the right track but there's a LOT of things that could still be wrong even given what you've posted. Please help us to help you.

Have removed the IP of the server, I have used this because jumping in to a single player game, making a smooth stone block and placing it resulted in a 1 line log file.
First, if you're asking for help with Java then just stick to that. Bedrock is a completely different animal and isn't even remotely relevant to trying to get a Java pack to work. It's apples to oranges. Please ask questions about Bedrock packs in the appropriate forum. This is the place for Java packs.
With that out of the way, the pack_format for a 1.16.x Java pack should be "5" though I don't think that's necessarily your entire problem. It might be part of it, but probably not all of it.
Honestly, given the information you've provided this sounds like a problem with your game, computer, or installation. If you've tried several 1.16-compatible resource packs that reportedly change the smooth stone texture and NONE of them work then that's a problem on your end.
If you'd like more help, please remove EVERYTHING from the pack that isn't 100% your own creation and upload it somewhere. This means removing all of the default files and anything from another pack. Even if it's edited or recolored. Given that the pack is called "Combination Pack" I'm guessing that that's not going to be possible but in case I'm wrong the offer stands. If you can upload the pack while keeping within the forum's rules, I can try to check it on my side to see what the problem is.
I have posted this in a number of forums, and this IS the resource pack help forum, there seems to be no distinction between resource pack help java to bedrock that I've seen.
I will make the necessary alterations to the format from 3 to 5 and see if that helps....

Since the installation is new every time a most recent update occurs, I can't see it being an installation problem.
Considering that every single texture in the pack performs as expected except for smooth stone, I'm certain it's not a purely my end problem. There has to be something in the coding somewhere.that it is specific to ONE TEXTURE ONLY. The texture file itself is not the problem, as have done testing with other texture files the same. That's why I thought it may have been a known issue, because it is only with that block type.
The issues occurred with all packs that I've merged the elements I've desired when utilized seperately. None of the pack is my own creation, it's just a collection of textures from other packs that I like, but without the one pack rules all problem with other textures.
But, Iwill try the pack format and see if that has done it.
Never Mind.... under the format 3 smooth stone had a different texture name, but doesn't use it, and trying to enter a blockstate or model to point it at the new filename (or even old filename) ruins every other texture in the pack, so I'm going through converting it to 1.16.... which raises 2 points for me...
The naming convention in 1.16 ( don't know if it's the same for 1.13 and 1.14 ) is retarded.....
and if I could be bothered putting together a database with all the filenames from now until the beginning of time I'd make a nice universal converter, but I really can't, looking at these naming conventions has made me wonder how long it would take to ensure nothing is missing :S It's so braindead it saps my will to play and makes me want to die renaming from one that made sense. Just ugh.

Changing format from 3 to 5 resulted in every single texture no longer being displayed.

What bit depth did you save your png for smooth stone with? Minecraft doesn't support textures with a colour depth higher than 32 bits.
