Acronym for Guy In Real Life. This is usually referring te guys who use female characters when they have the option to have male characters. This happens usually in MMORPG's but also other games such as Mass Effect. Do you think this is sick, gay, or perverted?
Depends on the intentions of the person. I often play as a female character for role-playing purposes which I consider fine. But then there are others that play female characters solely for the social benfits of having a higher chance of recieving free items and such.

^^^ Basically this. As long as you're not exploiting the avatar's appearance, you're fine. I've never been one to give a female character free stuff. Half the time, they're guys, and the ones that actually are girls usually aren't very nice on the eyes. Sometimes in roguelike games I will randomize my race, class, and gender, just to try out a setup I'm not used to. I'm not going to start all over just because I end up with a female character; the game plays exactly the same anyway. I know that's not the same as an MMO, but still.
I play characters of either gender; when I create a character I'm not projecting myself onto the character, but rather making a character that I think will look interesting based on the character's play style, often taking inspiration from tropes.
I like to mix it up, its fun to play the other gender sometimes.
For instance, my friend lent me the entire mass effect series(gonna play it all AGAIN :D) and my character is a female.

I usually pick the one that has the most interesting aesthetic... I'm one of those weirdos who will use a weaker piece of equipment because the 'stronger' equipment is ugly as sin. Same reason I'll play as a female character if her armor graphic is more interesting than the male counterpart (big example of this were Necromancers in the original guildwars... I hated the male armor for them, but the female armors were nice).
Of course, I don't lie about my gender either... if you have friends, you tend to help each other out regardless of gender.

... or they just want to play as a female character because they understand that it's just an in-game avatar and that the gender you play as doesn't matter?

People often judge other people by their in-game choices and behaviors. I am not speaking of my own opinion in the OP, I am merely stating the opinions of other people.
I have a friend who choose to be a female in Halo Reach simply because they have slimmer hitboxes or something.
This is also a valid reason I completely forgot about, it's the same factor in why I always play as Anya in Gears of War. Similar to how covenant elites had a slightly longer range for energy swords, yet their heads made for a much easier target. This was back in Halo 2 by the way, it's probably completely different by now.
When it comes to Roleplaying games, I always make characters of both genders, for the simple fact that, depending on the gender, some game options differ.
Take Fallout: New Vegas as an example. If you are a male, you have the option of sleeping with a prostitude by succeeding a Speech Skill Check of 30, while if you're a female, you need a special perk which unlocks additional dialogues when having to deal with the same sex.
Other than that, I do it purely for the aesthetics purposes. I never roleplay a female character, for the simple fact that I cannot roleplay a female in general. I've lived my whole life as a boy, I bet it's not that easy to think of what would a female character have done instead of a male character. But I always choose the female trainer in Pokemon games, for the simple reason that I find her more attractive.

What if the digital world was a part of our real life? Not that it isn't, or anything, just saying.

Some players will judge other players regardless of reason for the sake of being judgmental. Some nerds just don't like other nerds.
There's no problem with people playing whatever gender they want to in video games. We, as players, are responsible for not assuming that a person is male or female based on their avatar along.
I remember when I first started playing WoW with my wife we switched genders. When we joined our first raiding guild (I think this was in 2005 or 2006), our guild freaked out the first time we talked in teamspeak. The big burly man had a tiny girly voice (my wife has a very young sounding voice) and the little rogue had a baritone voice. Their gendernormative assumptions crashed with about an hour's worth of commotion.
I've had a couple of encounters in which guys have asked me whether or not I was a girl, and when I truthfully told them that I was in fact female, they didn't believe me. LOL
But anyway, back to the question: I don't really see the problem in using a character of the opposite gender as long as it isn't for perverted reasons. I used to use male characters in an online dancing game just because it was easier to get into the couple dancing rooms, which was a mode that I liked a lot.
There's also the fact that some people do it for aesthetic purposes. Sometimes, games tend to prefer one gender over the other when making special outfits, so some people choose a character of the opposite gender just for the customizations.

They had slimmer bodies. I think the actual hitbox was the same.
But to the OPs question: No, I dont think its sick or gay or anything. Perversion...maybe? If they were using it to cyber I suppose but thats no more perverted that the sex anyone else has.
Discuss.

Never thought about that. Smart.
I don't find it a problem. Its just misleading sometimes.

"Hey Cindy you playing Half Life? You disgust me!"

I would disagree with that.
