This system is based on LastOutput tag of command blocks : If you make a command block run a valid command each tick, like "/gamerule commandBlockOutput", the block will always detect it's false, so the LastOutput won't change...
Well, yes, it will change : The LastOutput also shows the moment the command have been run : From that, you are able to detect a precise moment in the real life, simply by running a command looking for the LastOutput :
There're tons of JSON things in it but, for example, you can detect if the LastOutput is "[21:47:13] : The gamerule is on false"
Of course, if the game is paused or doesn't run when the real hour matches the moment you programmed, you'll have to come back 24 hours later, but this is still such a nice thing to know.

You know what? I made a system able to test for real time! It works for 1.9 and upper, and it can test if it's 12pm or 12am!

If you want to make 12 hours playtime, I think you can test for it with something like /time query...
20 ticks = 1 second...
20x60x60x12 = 864 000 ticks

good point, I guess I misread the part about literally being 12 hours. I was thinking "12 hours" game time.
I would actually use a comparator clock. I would then set it for one tick clock speed. Then I would just use that's score to track the time
if you need the amount of ticks in 12 hours, do simple math.
it is a total of: 864000 ticks
This is much easier to achieve with a long period hopper clock. At least in bedrock.
