When I play on my main survival world, which has been going over many Minecraft versions, disenchanting items gives me no XP back even though it removes the enchantments. I tried in a fresh world and it does work, so is there some sort of game rule that got changed in my main world that would cause this?
I haven't tested it recently but the last time I tried, a couple of updates ago, I had XP orbs getting stuck behind the grindstone if I wasn't standing close enough.
It could be the fact that you may be using it in "old" chunks, try to set it up in a newly generated area. Good luck
Minecraft doesn't work that way.
Things that generate with the chunks will only be found in new chunks (ocean monuments were a special case, new ones could be generated in old chunks that the player had only passed through briefly).
Mob spawns and game mechanics only care about what version the game is updated to, not if you are in a chunk that was generated after the update!

I believe there is a gamerule that controls drops, which I believe happens to also govern xp orbs. However, that's absolutely global to everything; there's no gamerule specifically for grindstones or other functional blocks.
Perhaps it's like furnaces? I dunno if it was actually a bug (probably fixed already since I play in 1.12 still) or just how the system worked but I noticed that sometimes a small amount of items smelted would give xp but other times that same amount of stuff didn't give xp. Maybe you're facing the same sort of issue.

If by a small number of items you mean just 1 or 2 that would be completely normal, smelting 2 potatoes, for instance, is worth 0.7 XP which means that 70% of the time you get 1 XP and 30% of the time you get none.
