I wish now that I'd put the sword in the first slot so that it would always be the first thing I'm holding when I start up. But it's too late now; I'd constantly be hitting 9 and fending off creepers with a shovel.

It's not that I don't like change. I've gotten too used to my current hotbar. When I need my sword, my finger immediately goes to the 9 key. If I put my sword in 1, my finger would still unconsciously go to 9, since that's where the sword's been for so long. Conversely, I'd constantly be trying to use my sword as a shovel. It's best that things stay where they are.
I've added a poll, which I'm actually very interested to see the results from.
Personally, I use the scroll wheel, mainly since it's already under my fingers.
Use of the scroll wheel is also mainly why I put the sword next to the torches, pick next to the cobble, and shovel next to the sand/gravel. I've noticed a person's hotbar setup seems to be highly dependent on whether the person uses number keys or the scroll wheel.

Why don't you add a pole option for 'Both'? I personally use the mouse wheel for casual selection, but in an emergency I press 1 for my sword.

For the purposes of the topic I think I will keep it as "which one do you use most?"
In my experience, when given an "other" or "all of the above" type option, people basically abuse it, and you end up with more "both" than either definitive answer, which is not good.
I like to keep things organized in my inventory, but that's just personal preference. I don't need to keep things consistent or anything because I use the scrollwheel.
1. Pickaxe
2. Shovel
3. Axe
4. Sword
5. Bow
Slots 6, 7, and 8 are used for whatever I need them for.
9. Torches
Arrows are kept in the top right slot of the inventory. Extra pickaxes go in the slots above 1. Extra shovels go above 2, etc.
If I don't anticipate to be needing an axe, I store the axe to free up inventory slots and go with the following:
Remember, scroll wheel makes it easy to have things wherever in the hotbar you feel like at the time.
I have special configurations for mining (not spelunking!) and for construction.
Construction Config:
1. Appropriate Tool for removal of building material in event of an error.
2, 3, 4, 5, 6, 7, 8: Building material
9. Torches
Mining:
1, 2, 3, 4, 5, 6, 7: Pickaxe
8. Cobblestone/stone for patching holes, depending on whether I am digging for a base or quarrying/strip mining. I usually quarry just because it happened to be what I did first time I built a mine.

Pickaxes in the config can be of any type, but there is never more than 1 iron and 1 diamond pickaxe in the hotbar at a time for the mining configuration.
Funny thing, I used to organize my quickslothotbardoohickusthingumbob almost EXACTLY like that, only the sword and bow were switched and I had torches in slot 9.
