I lost 22 diamonds while lighting up caves, I feel into a waterfall then flowed into lava, I couldn't fight the current and fell into the lava, needless to say there was a lot of cursing that went on because I couldn't find where I fell in and lost everything
So I was on a great towny server. My town had a very jerky mayor who broke into my house and stole my goods. (Even my beacon. I killed a Wither to get that thing.) I left town on the spot, confining myself to my mesa base.

Come to think of it, I mainly play fighting-based games.
After finding a very cheaty extremely high fortune level pick at spawn after a dead admin dropped it I found my way to my mine and mined up some diamonds I had been saving only to find that I crashed the server... I did manage to get the ore later with another pick but the server rolled back and I lost the inventory full of diamonds I had acquired.
While I haven't lost diamonds (Don't mine for them often), I have lost plenty of Mithril in the LOTR mod (Which is harder to find than diamond, 2.2-6.1 ore blocks per ten-thousand blocks depending on where you are, versus diamond's 7.6 ore per ten-thousand) before. I had been mining in the Misty Mountains, one of the two places Mithril will spawn (2.2 ore per 10k blocks), and I had fallen into lava. Lost 15 mithril ore, a mithril sword, and a mithril pickaxe, as well as 3 stacks of elven steel (an upgraded form of iron, basically).
After mining from a Savannah ravine, I had my Efficiency II diamond pickaxe with me ... It was night when I exited the place when I started fighting creepers. Until a lag spike came and majorly forcing me to move backwards without knowing the ravine behind me. And there we go, I fell and lost my diamond pickaxe, 7 new diamonds, and probably a stack and a half. I didn't find it because it was far away from my base. I didn't bother because I currently have a stack and extra 13 of diamonds and tons of iron. I remembered this because this happened yesterday. ^-^
I have returned after all these years and I'm internally dying inside whenever I think about the posts I've done prior to this current year. Just don't look at them.
fell into a lava river to mine iron ore, got back up diamond gear and ventured into the nether just to fall into a 38 block pitfall (golden horse armor, 6 saddles, 7 diamonds and all the nether wart in the area fall into lava), Pigmen picked up my weapon and aromor and for the next 90 mins i fought to get them back, each time either getting over ran by them, or ganged up on by wither skeletons and blazes. i gave up, back to mining for me.

While digging an extremely long tunnel for an underground railroad I inadvertently fell into lava after digging the block beneath me. Not only did I lose an inventory full of rails, ores, etc. But also, ALL my diamond tools, weapons and armor. Most of which was maxed out in enchantments including Mending on everything.
But such is life in Minecraft. Back to the fish farm I went.

When I had finally found a diamond ore block, I mined & the diamond just plopped into a pool of lava I couldn't see & then I fell into the lava because I was standing on the diamond ore when I was mining it.
I had extremely good luck when I mined into a ravine near my house... I dropped down all the way to y 11 and right away found an 8 vein of diamonds... about 5 minutes of stip mining later, I find a 10 vein. I decided to put my luck (getting pretty lost in the process) and found emerald ore. At this point, I'm like, "how lucky can I get!?". I see more diamonds on a ceiling of a lava pool. I build a cobblestone floor above the lava and mine the diamonds, only to be suffocated by gravel and at 2 hearts. I step back out of the gravel on low health and a completely random skeleton shoots me dead from out of nowhere. I lost all of my diamonds and emeralds, and I couldn't even find my way back to my stuff because I got so lost mining.
What's even worse, is that I went back into the same cave found more diamonds, and lost them for a second time. I had to take a break from Minecraft for a day or so, but now I'm back lol.
I was searching for diamonds one day when I found it, after hours of searching I found diamonds! When I went to mine them I accidently threw my iron pickaxe in the lava do I couldn't mine them, I went back to my base and got another iron pickaxe. I mined the diamonds but immediately after get blown up by a creeper, I go back to try to find my diamonds but they fell in the lava.
Was transferring all of my items between an old base and a new base including my stack and a half of diamonds. The quickest path to the new base was through the Nether, and I ended up being hit by a Ghast and then falling into lava, losing all of my enchanted gear and diamonds. I never played that world again.

Bring an enchanted bow to shoot the Ghasts down, the necessities to survive in the Nether already exist. Unless you get a bad spawn (i.e stepping out of a portal atop a cliff) you shouldn't be having this much trouble protecting your expensive gear.
Mojang hasn't made this game that hard and they likely never will.
Of course if players are new to the game then they're going to make a lot of mistakes, but a seasoned player would know the tricks to survive in the Nether properly by now, I've just been in the Nether last night, with netherite and diamond gear and not died while finding the location of the Blaze spawner.
And I'm not even a pro at this game, I just happen to be experienced and competent enough to watch what I'm doing.

Tbh at that point I'd just restart the world if I hadn't backed up an earlier version.

I've learned that restarting worlds over and over again is a waste of time, and it is better to make backups where possible, and use them where appropriate, i.e when somebody griefs your world after disobeying an anti grief rule, or if a problem of "Unable to connect to world" persists.
Java PC and bedrock edition sized worlds have more than enough resources, why worry about losing just 2 or 3 diamonds in lava? I could see why people would want to start a new world if they lost an entire chest worth of diamond gear or even one stack worth, but even then scrapping your builds and the rest of your work for the sake of just 1 stack of diamonds lost in lava is a bit over the top in my opinion. It's replaceable and you can just learn from the mistake, if we don't learn from our mistakes we will repeat them which then defeats the entire reasoning behind restarting MC worlds.
this happened right now and i was hopping around a cave to go up a waterfall and i fell into a lava pit that i couldnt get out of losing my netherite pickaxe, sword, my 12 diamonds, my full enchanted iron armour, and my hot bar items ive had since day 1

When i fall into lava everything is just sitting next to the pool of lava but my diamonds.
