In the first world I ever got to the End in, I lost a full set of diamond armour in the Nether to a cliff and lava after accidentally enraging a huge mob of zombie pigmen and running like no tomorrow.. It wasn't that I jumped, I tried to make it so I'd die away from the lava and the pigmen hit me so hard I got knocked off the cliff.
Then, I got to the End later with more diamond armour and the dragon knocked me in the Void. Needless to say, I deleted that world pretty fast.

I have only lost diamonds once,
I was transferring items from my starter base to a newly built storage system, In the middle of the night, with my sound off (Was listening to music) I started a fight with a 2 skeletons, I had just defeated them and noticed one of them had dropped its enchanted bow, I walked over to check what enchants it had, meanwhile a creeper had snuck up behind me, I had lost a little health in the previous battle and the creeper blasted me skyhigh - killing me instantly.
I respawned back in my starter base and immediately bagan running back to my death point - which was about 5 minutes away. I arrived to see my all of my diamonds, iron, gold, coal ( in block form), my enchanted diamond armour, my enchanted diamond tools and a worn out pair of elytra disappear.

Diamond pick fortune 3 efficiency 4 and a mending book added
about an hour into exploring a cave, completely lost and run into a room full of skele’s and a witch.. splat goes the noob and 8 hours of fishing for mending books go poof!

Well myself i was on a trip to mine and i got about 7-8 stacks of iron , a few blocks of gold and 32 diamond + 2 echanted picks with mending + all armor.
Naturally i started running back to my home when i fell into this pool of lava and burned up
I've never died with diamonds o.o
Rather I... umm... /clear myself instead of /kill Deleting nearly 3 stacks and a fortune 2 pickaxe... Don't ask why, I still don't know XD

I was mining obsidian when I got blasted out of my water stream. The creeper didn't kill me, the lava I was blasted into did.
