Gingerly making my way across a cave by building a bridge about 5 blocks above the lava/water floor. Suddenly a skeleton pops me from out of nowhere. I fall into the flowing water...no problem right? But I can't escape the torrents and I'm frantically trying to figure out what's happening...then I see I'm crouched. But instead of staying crouched and sticking safely to the edge of the water/lava edge I frantically decide to uncrouch and leap for it. Poor idiot (me) never had a chance. I lost full diamond armor with various enchants, and a quarter stack of diamonds plus complete diamond tools. I felt as if my puppy had been killed.
Mine is so sad. I was in the mines for a long time, trying to get as much things as possible. I got at least 13 diamonds. I was so proud. Since my iven. was so full of cobble I went to a lava pool to clear it out. Thats when I got hit in the back by a skeleton and died in the lava with all my stuff....... Lesson learned constantly go back to your house to put your precious items you found in your chests.

You had enchanted diamond armor but not a water bucket?
A long time ago I was running through a cave, lighting it up, when I suddenly fell into a pit of lava, which had been concealed under a 1x1 hole on the other side of a 1 block rise - and emptied my water bucket under the lava, with no way out. I don't know exactly how much I had on me but this was before I used Ender chests, but made more room in my inventory by compacting resources into blocks and I only mined what coal I actually needed back then so I could accumulate quite a lot of other resources; I know that I had at least one full stack of iron blocks (576 iron) and some diamonds - plus my gear, which is all diamond and enchanted to boot (losing an enchanted item, especially prior to 1.8 or in 1.9 (Mending), is a much bigger deal). Needless to say, I reverted to a backup copy (replacing my gear is no issue at all in terms of diamonds, and at the point I am at now I have enough enchanted books taken from chests to replace everything at least once).
Otherwise, I don't really remember the last time I lost any items, the last time I even died at all was nearly two years ago (in that case it was a creeper blowing me up, the only risk of losing an item was if a zombie picked it up and killed itself somewhere else).
Also, for a time I used a backpack mod, with 4 double chest sized backpacks used to carry resources, and used Fortune to mine everything - in that case if I had ever died I could have lost hundreds of diamonds and tens of thousands of resources. Even more recently I've mined over 7000+ resources during one play session without using Fortune (an Ender chest can easily hold a couple play sessions worth of resources).
I had a ton of stuff, I was exiting the ravine I was in. I pillared up and I was almost out. Then all a sudden, skeleton shoots me and I was basically insta-killed. Ragequit.

I never lost diamonds, because I always secure the way I came from while mining, going as far as sealing off unexplored sections with glass and lighting everything up.
Also, when I find diamonds or other valuables, Iusually get out soon after to stock them in my castle.
Last time I was on my friend's server, I went into his branch mine with him.
We came across to a ravine, where I spotted diamonds. I immediately mined them up, and there were in total 4 of them.
And then I saw that I was standing on a redstone ore, so I thought, 'I'll just mine these too,' and I found out that beneath the redstone was lava. I was too excited so I forgot the number one rule of Minecraft: Never dig straight down. My friend had a good laugh at this and cheated back the diamonds for me though.
For the next few days on his server, I never stood on an ore when I mine them...
Was strip mining, and I was digging in another strip, and my pickaxe mined a bit too quickly, so the lava broke through before I could realize. It flowed a bit more quickly than expected. Around 24 diamonds gone. Goodbye. Nada. Nope.

So I decided to do the obvious thing and cover it up.(keep in mind I had around 40 diamonds full enchanted armor and tools with 3 additional pickaxes) so after I covered it up I went into my inventory to move something to my hot bar. Before I could even exit my inventory a creeper came up behind me, exploded and broke the blocks I used to cover the lava and then I fell in and all my valuables burnt to a crisp.
I was making a short trip to the nether. All I needed were a few blaze rods, so I didn't take the time to go deposit my diamonds. It was at that moment I decided to patch a nearby lavafall for some reason. I fell and got caught in the lava flow.
Wanted to bang my head against the desk when this happened. So many terrible choices were made...
Started a server for friends and decided to go to the Nether for the first time. The server was loading the terrain and it timed out and kicked me. I rejoined and it spawned me at the top of the ceiling and i fell about 400 blocks to the ground below. I had a diamond pick and diamond sword.
So we have all been killed by creepers blowing up us and our newly mined diamonds or sometimes its just lava. Share your story with us.

i didnt really die, but i mined some diamonds and decided to throw out some junk in the lava, accidentaly throwing my diamonds in aswell...

Lost an uber diamond pick to a lava pit I created to deal with trash. I learned to remap the Q key...

Doing alright, trying to find my way to the surface, keep ending up going down, I decide to dig straight up, and pillar out.
Between me and the surface was an underground lava lake...
