For a while I was afraid of the endermen, but slowly, I learned more about them, and they weren't so scary anymore. I think if notch wants to create something we are all truly scared of, he needs to create some kind of unknown horror, a true mystery. Why do you think mankind is afraid of the dark? Because it's afraid of the uknown.
But it's difficult to create the unknown when so many people on the internet can dig up so much information. So what's my solution? make it's attacks completely random. Sure, you could dig up information on what it looks like, how much damage it does, or what loot it drops, but you could never know when it attacks.
You could either become paranoid, constanly preparing for the day it does attack you, or relax, and be unperpared for the day it comes. Both result in fear.

You can't always make it have a random attack move. They would take too long to script, and would eventually max out in possibillities... Have you thought of how hard it is to make something attack with a random move by code?

No, think of it like it spawns at random, but it always spawns near you, and thus always attacks you when it spawns.

The Endermen at the moment aren't incredibly scary also because of a few bugs. Look at creatures like the Ghast, even though we know all about them, they are still potentially scary creatures.
The Enderman has a couple of advantages over the Ghast that keeps them scary:
1) The Enderman is darker and blends in more with the darkness.
2) The Enderman is smaller, so he is not always easy to see, but is the appropriate size and lankyness to be creepy.
3) The Enderman is passive ,but provoked through strange means.
4) The Enderman can pick up blocks. (hella creepy)
5) The Enderman can teleport
With that, The Ghast has some advantages:
1) Ghasts can create explosions and do more damage
2) Ghasts cannot be avoided like the enderman.
3) Ghasts make scarier sounds.
However, the Ghast will lose the advantage of sound when Endermen are given their own sound files, which will probably by even creepier than Ghasts.
I understand your concept of something random and very scary and paranoid. But Minecraft is not meant to be a horror-themed game. While some of the concepts are creepy, they are not meant to make all of Minecraft creepy, it is only meant to add a survival aspect to the game. The type of mob you are suggesting would be more of a random even such as a strange fog weather type than an actual spawning mob like the Zombie. While the idea is good, I think that it would be more suited to be added into a mod of the game that is meant to turn Minecraft into a horror-themed game. I am not sure if such a mod already exists.

He got so fed up, he went and killed them all, but everywhere he went after than, all the animals just stared at him. Squid too. So he stared back, and then, suddenly, the squid he was staring at swam straight away as fast as it could, the other squid watching it now, not my friend.

In order to have what the OP was asking for, it must be something we don't know for sure about. If it's just another mob, sooner or later we'll know everything about it, with screen caps and a detailed analysis of what it does and how to defeat it. You simply cannot have an Unknown Horror if it's known. So, it follows that The Horror must be something that cannot be confirmed or denied with certainty.
Does The Horror exist? I dunno. Do you want it to? It's there if you want it. Unfortunately, it may also be there if you don't want it.
