Currently, only specific items have any actual function in the offhand slot, which are just items with a right click action such as the shield, bows, food, blocks and throwables. Items like a sword, pickaxe, axe, shovel, pretty much anything that is meant for mining, has no function in your offhand, and basically serves as an extra slot. I think this should be changed.
When you go to left click, it depends on what your character is currently looking at at which hand it will use. For example, if you have a shovel in your left hand, and a pickaxe in your right hand, it will use the shovel in your left hand to break the block if it is something such as dirt, however, if you are facing something such as stone, it will use the pickaxe in your right hand to break it. If you are holding 2 of the same exact tool, it will use your main hand.
Another example, if you are holding a pickaxe in your left hand and a sword in the other, and you go to hit a mob or player, or just swing at the air, it will attack with the sword in your right hand, but if you go to mine something, it will use the pickaxe in your left to mine the block. Holding 2 swords will result in your player using the one in your main hand.
Holding both a sword and axe at the same time, both of which are designed for attacking, will result in attacks being with whichever one is in your main hand, but mining something like wood still being the axe, regardless of which slot it is in.
Basically, what I am saying is, if you are holding a tool in both hands, holding left click to mine will use whichever item is meant for mining the block you are looking at.

that sounds like a terrible idea - suppose I accidentally scroll a little to far? now I have to scroll through the entire hotbar just to get to the previous item.

Like if I have a sword in the main hand and a tool in the offhand and I try and do a sweep attack on an enemy but I wasn't pointed directly at it, but instead at a block behind the mob. Instead of the sweep attack, the game prioritized trying to mine so I take damage from the mob.
Beyond some edge case scenarios or classes of items that need to have priorites determined (does a Splash Potion in the offhand take priority over a sword in the main hand? Is it dependent on if I am targeting an enemy or not?), it seems like it would work but is kind of unnecessary when the default key to swap them is super convenient as-is.

Is it dependent on if I am targeting an enemy or not?), it seems like it would work but is kind of unnecessary when the default key to swap them is super convenient as-is.
A splash potion is a right-click action to throw, so holding that while also holding a sword has no effect, and will still attack with the sword. Holding a sword and another item depends on if you are looking in the general direction of a mob at which it will swing. Tapping left click while looking at nothing, at a mob, or in the general area of a mob, will swing the sword. Looking directly at a block and tapping left click will use the pickaxe. However, HOLDING left click will result in using the pickaxe regardless of a mob being present or not.
While more to do with the offhand would be good I think adding a new slot to assist the offhand or main hand would be good.
Like the quiver, a potion bundle/pouch for example. I don't know if scrolling (separate scrolling offhand and main hand modes or an off hand inventory from a backpack maybe).
Another option could be more slots like with MineBlade Battle Gear, which had 3 slot in both the main/offhand to switch between say torch and a pick, sword and potion/shield, or bow and arrows (or whatever you want to customise it with). Obviously it could be worked to suit how much you need in Vanilla like 2 slots, 1 for Torches & a Pick, while the other could be for swapping out a Shield & Sword so you don't have to rely on using Slot Number keybinds for the main hand or your Offhand binding to switch out between situations when you say are mining and not getting attacking to when you can defend yourself from mobs.

Now, this might be controversial, but what if you could switch equipped items in each hand independently of each other.
The big problem I see is how moving the equipped slot would work if both buttons or keys are taken up by a different hand.
