Similar to how console minecraft has different prebuilt worlds you can generate, I think it would be interesting if the game could have several other structured world modes that impose some very specific functionality. I have a few ideas, but this is one Im more interested in at the moment.
The idea of this mode is to create a survival world where the player only has one chunk, 16x16 blocks from bedrock into the sky limit. Either it could be completely random whats in it, or Mojang could custom engineer a chunk with as many unique features in it.
I would like to see the one-chunk mode still have nether and End access, but I think they would have to be regular sized for the player to really make use of them.
Another thought is to make it so the player has full access to the overworld to mine and explore for materials, but at nights the player would be instantly killed past a certain time, if they are not inside the "safe chunk", and maybe skipping to dawn could be disabled to force the players back to base.
That sounds interesting, and also sounds like something easy enough to do using command blocks. Actually, you might look at putting something like this together in command block and float it out there for the MC Community to enjoy and give you feedback.
You MIGHT be interested in the Captive Minecraft series by the Farlanders. Do a web search for "captive minecraft farlanders". You can watch videos of Logdotzip playing as well as others and get an idea. In short, you start in a world with the world border set at a single block in size. You CAN NOT MOVE! The only way to expand your world is by getting achievements (or advancements, depending upon what version in the series you play). As you make advancements, the world expands JUST a little bit. That's not quite your idea, but it sounds pretty close and it may tickle your fancy. You can also see how they did it, then work out your own riff on the theme like you describe above.
EDIT: A glance at their website comes back dead. https://www.reddit.com/r/TheFarlanders/ has not had any recent responses. You may be able to get a copy of Captive Minecraft IV, but at least check out the video for ideas and perhaps you can get a copy from PlanetMinecraft or other resources. Even if The Farlanders have moved on, Captive Minecraft seriously sounds close to what you describe in your OP.

Yeah, command blocks would indeed make this possible in PC one way or another. Im not sure what the simplest way would be to prevent people from breaking out of it however, especially when nether portals come into play. At the very least, I can shave the bedrock floor down a layer or two and make it a solid floor and then use those invisible barrier blocks to surround the entire thing, and block off the max height layer with them so people just don't build up and jump out (In other words shave a layer of bedrock off to make up for the ceiling height being lowered by one block.)
Not sure if there's a way to edit settings unique to that world though to prevent additional land or etc from spawning, and then to keep players from nethering outside the single chunk.
Another similar world mod I have in mind is a take on a fan created LEGO theme, I think called moon base. Usually a collaborative project, builders create modules that have doors at set locations, so when everyone brings their module in, they look like they are part of one larger base, with tunnel doors connecting every base unit. Players would be given 4 16x16 chuncks, arranged 2x2, and players cannot exit the module except through specific holes in the edges of the four chunks under their control. When players launch this mode offline, they have access to the outside world to gather materials for their module. When they load it in online mode, they will be joined by up to 8 other players and their modules which are placed in an interlocking manner, and maybe some random creations by other players to explore in.
It doesn't say, but I'm sure it will work in the Nether. You'd have to test it out. But if it says "Thou Shalt Not Pass", there are tricky ways you CAN, but they are not simple or easy.

It is non-trivial to pass a world border, even if it is 16x16.
