Regardless of the crafting recipe, command blocks could a positive addition to a vanilla server if there was the ability for the server owner to decide what commands were able to be used by the players that would best fit the type of server and play style of the respective community. This would allow players to make wonderful mini-games and such, but not allow havoc to be caused. So given the server owner had control, and could decide what commands were and were not able to be used, command blocks could be a great addition to vanilla smp!

Not necessarily, and again it's with respect to each individual server and its goals in regards to desired game play style. And with the before mentioned situation if the goal was better met without command blocks then the admin could specify such and it would be as the vanilla servers are now. But there comes a point in vanilla survival when it really isn't "survival" anymore. Once fully enchanted diamond tools and armor are obtained, which doesn't take more than a few real life days of playing survival, then it turns more into creative game play, with the only main differences being that one must obtain the building materials from the land, and there is no flying. In that case, and server play style permitting, a whole new level of mini game development and such could be introduced and add great new variety to the game play, which some communities would thrive on and their game play would be benefited from greatly.
I would support if it can only /testfor , /testforblock . and do such things that wont affect gameplay ( like /give and summon ) and are used for redstone circuits ( like /scoreboard )

I like the idea that you thought up of an idea that there should be a command block recipe, and there should. But this recipe, it is sadly too easy. If you make it more OP, maybe I'll support it. But for now, no support.

This is an awful idea. Adding these things to Survival is just not good. I could give myself infinite diamonds or Wither Stars or whatever. They should stay creative only.

Ironically, command blocks can't even be used in Survival (try placing a command block in Creative, then switch to Survival and try to enter a command), making this recipe useless other than making a block you can't break, which is still enough to warrant not adding it even for decoration.
There is also a reason it isn't even availabe in the Creative menu, as mentioned above (you can be in Creative but not have cheats/commands enabled, meaning you can only access blocks/items that are available in the Creative inventory).
They can be used in Survival, if you set them up in Creative, which is what I do. But once you switch back to Survival you can't change the command that's been entered unless you go back to Creative, that is true. So if this was SSP, and you didn't have cheats enabled, crafting them at this point would be worthless. But if they ever decided to add a crafting recipe, this could easily be changed as well.
And they are used in Survival on SMP servers for various things, like auto-banning people for certain infractions or /tp people to starting points or giving they certain items (like Bingo cards) in games--it's just only OPs can enter commands into them.
I think if you want to use them in your SSP, fine, but don't ask them to change the game to make it so you don't have to feel like you're "cheating" to get them. They actually don't do anything that you can't do already with cheats enabled. So it's not something i think would really add anything meaningful to Survival mode if they were craftable. If you want access to those commands, you can play with cheats enable. There you go. If you like using the command blocks themselves, like I do, OK. Use them. They are fun and they give you something else to interact with in your world. I love them for /tp to locations around my vanilla world while I work on a larger network to connect everything. But I really don't think they are something everyone playing Survival needs or wants.

and if you misplaced it.... You can never remove it
Plus if a troll in a server got it... Well... its pretty bad...
