Eh, I still say books. Like Wedhro said, the only use they have on their own is crafting something else, which even then is only decorative and is pretty expensive. 3 canes to make 1 paper, 3 paper to make 1 book (9 reeds so far), 3 books to make one bookshelf. 27 reeds and 6 wooden planks for a single bookshelf. Sugar or milk could be argued similarly, but the crafting isn't as expensive for cake.
EDIT: And gravel still can be used for staircases in caves, and getting flint. There are a lot more useless items.

Every item has a use. If it didn't it wouldn't be in the game. Things like the dead shrub are for aesthetics; it's still a use.
However OP asked for the most worthless item, not the most useless. The most worthless item would be every item unless you have some sort of value system for the game's items.

lapis lazuli dominates as the top worst item ever. it might be good for wool but if used for anything else not related to TF2 or TR0N then it is a shitty piece of decoration.

Transport for what? Are you shipping furnaces from one base to another? A storage cart would be much more efficient. They don't do the job they were intended to do (they can't push other carts), so they're totally useless.
Decoration doesn't count as a use. Decorative things are, by nature, generally useless. A good example in MC being bookshelves.
IMO Bookshelves are twice more useless than powered minecarts.

Decoration, I think, is really in the gray area between usefulness and uselessness. I personally don't count it as a use, but most of the things I make in Minecraft are function-over-form. I've never built a bookshelf, and I just crafted my first painting yesterday (got the Donkey Kong one first try, hell yeah!). Lately I've been going through my world, renovating all my structures to be more aesthetically pleasing, and part of that is phasing in "useless" decorative objects, so my stance here is up for change I guess...
