Is there a possible way to make a "grief" tag, such as:
/grief which tags a player saying:
Grief: #
for how many times they have been tagged, it would be for op's only but members of the server could vote for and against it, and they could vote for it to be tagged.
Fair (and effective) anti-grief is paid-only servers + perma nick bans. Simple and effective.
As for free servers, just have lots of ops + 10 minute back ups.
I think this is the best way to stop greifers in the future, i doubt anyone who paid for the game is going to bother being immature and pathetic enough to greif
Inevitably some do, however the ratio of paid griefers to free is tiny, so once you ban them they can't just sign up again.

That would get abused FAR too easily. Griefers (for example) could tag everyone they see without the other players knowing, and that would repeat until players have a massively high number where nobody would trust them anymore.
As things are right now, you can just keep an eye on players every so often and then /banip them if you spot them griefing. The admin blocks blocking the spawn point is very useful as well.
From what I have seen the idea of having a maze made on unbreakable blocks works fairly well to weed out griefers. Most griefers will not take the time to figure out a maze and will just switch to a different server.

If there could be some way to set building permissions, we wouldn't need cages, overflowing ban lists, etc.
Admittedly, that means more work on Notch's part. ._.
But it sounds really simple in concept, and probably has been suggested a hundred times by now. Then you wouldn't need a ban or a "/grief tag".
Just two cents that I feel are on topic but out of place.
1. Add a budy system, only you, your buddies and ops can delete your stuff. (this might cause problems because griefers could take advantage of people afk and block off the spawn)
2. Fixing the problem with the above, if the player is surrounded by blocks from another player from all sides they're allowed to pass through them like if they were air (unless they're surrounded by unbreakables built by an op)
That would take care of most of it, and obviously these:
/votekick
/voteban
Optional commands that can be used by the players if the server admin allows it on the server config.
I'll post it again on this one, don't create anything you want to keep on a public server where anyone can destroy it. If you want people to see it, take screenshots, only build stuff you are OK with losing on public servers.

This undermines the spirit and purpose of minecraft in just about every way, in my opinion.
It's a sandbox game, building permissions undermine this.
Although an OP version of every brick would be good.
Although an OP version of every brick would be good.
No. All OPs would build using those blocks only (it already happens with just the one block), essentially creating a permission's system and ruining the game.
I've never had a server where every op restricts everyone else's building areas with their own op blocks, but only for their own buildings to prevent them getting griefed. Though I'm an op on at least one server, I don't really use that block when constructing stuff, but only for outlining main things (like underground rooms or bridges) to prevent griefers drilling through it and possibly flooding the room(s). (I wouldn't use the op block for building anyway; it would look ugly.)
I also disagree with ops having their own library of permanent blocks. There's really no need for that. It's basically cheating!
The system I suggested works well on garry's mod, I don't see why it wouldn't work here, the games are both sandboxes and both about building, it would work just fine.

Now, if players could do that, it might make more sense. But even then, a vote system is much better for that.
In the end citric is right, there really isn't much that can be done now aside from lots of ops, and map backups.

well then the server owner can sort that out? or we could just make it so you always have to have at least 2 people agreeing with you.
Edit - Woops... BBCode messup.
Choose your ops carefully, then. If you've got a bunch of mature ops who actually understand how to op, you shouldn't have problems.
Admittedly, that means more work on Notch's part. ._.
