Yeah, it started up for me just before the update really. After Mining out my Water Temple and working around it I've had the error show up more these past few days than I've ever had it happen over a year and a half.
It even happened again after the update too, so the update didn't fix it (Although they never mentioned fixing it in the Patch Log anyway)
When it does happen to me, it seems to always happen when I'm working/building in or near a lot of water, and after about 30 minutes of not saving manually.
So I've been manually saving frequently. But I've also noticed that even after saving a dozen times later, things start to bog down (Save process takes a unusual long time, blocks mining animation seems to lag, removing and placing blocks starts getting laggy) and then the error shows up. So after about a dozen Manual Saves, I'll exit the game completely and go back in.

Happens more now when I manually save. I keep forgetting to save manually after 10 minutes of play, and if I go 20-30 minutes and then save while working in water areas I get the error when saving.
I'm still hesitant to enable Autosave because I've only had bad experiences with it in the past and haven't used it in over a year. Maybe I should enable it and autosave every 10 minutes.
Seems water and Glass is my enemy... I just finished placing some Glass blocks around the border of my Cleared out Ocean Monument and removing the sand walls using gravity on a torch. Got about 20 or so minutes work done replacing sand with glass and forgot to save just now and got the error while trying to manually save... Grrrr! Since I've been working with the water and glass, it's been a nightmare these past few days.
I just want to finish decorating my Guardian Farm area and get away from it for awhile. Nothing but error problems slowing down all my progress. >:(

My server (which, by the sound of it, usually has it worse than everyone else) has been online for 3 hours and counting without issue. Fingers crossed...
macbros - a friend of mine also experienced increased instances of the crash when dealing with water, although not on my server, so it's not just you.

Playing since release, I've just had the "Failed to Load" issue happen to me for the first time. All I was doing was exploring the world so I could hang the fully-uncovered map on an item frame. Oh well. :^/
Edit: I'll add that my issue could also be related to water, as I was riding in a boat with new chunks generating around me.
I actually tried pushing the game to the limits to see if it is a memory issue. I did this by loading a normal world in creative and building a large, fenced-off area. Within that area I placed tons of item frames (with the fully-explored maps inside), paintings, redstone clocks, firework-shooting dispencers, and, of course, as many passive/enemy mobs as I could.
The framerate was awful, but the game did not crash.
Sure enough, though...as soon as I go back to my survival world, it is always one spot that causes the crash for me - a large lake separating the eastern edge of the map from the center. It was here that also I encountered the issue for the first time (mentioned in my previous post).
Trying to cross it both by boat/swimming crashes me. Every time. I'll investigate more and hopefully get some specifics that I can report on the bugtracker.

I believe i found out a possible way on why it does this. it seems to be due to a possible rendering problem that causes a fatal error because at a common chance my game crashes of the same problem either by pressing 'Select' button to access the player menu or the whole world no longer generates chunks correctly, like i get into a new world and sometimes within the first 10 minutes i come across chunks that are not rendered making you able to see what is below you but no matter how long you stay there or how far you are, the game crashes because of it. There was one time i was playing this with a friend, i try to access my furnace and the screen completely corrupted and hard locked my system because it rendered incorrectly so I think this maybe the case on why it does this. Also it could be that minecart with chests in mineshafts do not spawn right possibly due to cannot render correctly and causes it but I don't know, it's possible that could be another reason
Of course it's also happening on my server when the host is completely alone in the server, on a highly elevated platform in The End dimension, so water isn't the sole cause. Probably just one contributing factor.

Now my older worlds are getting "failed to load". I can play only if I save every 5 minutes. I tried 10 minutes, but "failed to load" message greeted me. My PS3 MC is not playable. This stinks! I'm back to my PC version. I can't get on the PS4, because the wife has it. Looks like the PS3 is going away when the new PS4 is released. Best wishes!
Wow. This issue is definitely more prevalent for you than for me. I've only had it happen three times, but not since. This might be a longshot, but have you tried deleting and then redownloading/installing the game?

This problem actually started doing this ever since patch 1.21 that is caused by a similar issue, trust me at the 1.21 patch, I have been getting 'Failed to load' quite often and over the latest updates I got more 'Failed to load' too common
4J comment on JIRA MCCE-441: "You should be able to load the world save and exit and then reload it for exploration.
There is a known issue with newly created worlds in the recent update that will cause the issue to occur if you explore the world enough after creating it."
I don't know why i'm so lucky to have this problem that new worlds "FAIL TO LOAD" and old worlds are having the same problem.
Following the suggestion, i have deleted MC and it is downloading and updating now.
Well after the game restored, I can play my old worlds for more than 10 minutes. I have not tried generating a new world yet.
The quote from that bug is aimed directly at users having the issue when creating new worlds and exploring them.
The issue being caused my older more populated worlds is not the same.

Hi, I have been having the same issues with the "Failing to Load". I have the VERY basic PS3...I believe 12g, Not alot of memory since Minecraft is the only game I have for it. (My kids got me into it). I do NOT have it downloaded but use the disc. I was having the "Failing to Load" issue before the new update, but since the update, no matter what I do, and I have disabled the automatic save, it crashes. I can't even play it. I thought maybe it was a memory issue and deleted everything on my PS3 that I was not using. (Even Netflix) But it didn't help. So I have no worlds I can play on. I have to create one and every time I start to play, within 30 minutes the game crashes and I lose everything. I am EXTREMELY frustrated as I bought the PS3 and Minecraft disc so I can play with my kids online, and now I can't even start a world. Any suggestions or ideas when this will get fixed???????
BTW - I am playing online, I play regular texture, single player --- on easy mode and survival (have never played creative (although the kids do..LOL).

Just so you know, if you do indeed have the 12gb PS3 which, I think, was backwards compatible (could be wrong), you could sell that for probably more money than a 160gb PS3 Slim. Might be worth considering if you never use it to play PS2.
He's referring to the 12GB PS3 SuperSilm model, which is not backwards-compatible with PS2 games.

Macbros & I are trying to get 4j to implement a 10-minute autosave as a 'band-aid' on this issue, until it's fixed.
A 10 minute autosave interval could prevent an hour or more of lost progress every day, at least for 24/7 servers.
And if you haven't already, click here to go to 4j's Bug Tracker, and vote, watch, & comment (preferably just once) on the issue
