Is it possible to change Block- and Itemmodels with McPatcher?

Sadly no. MCPatcher was discontinued not long after the models system was introduced into Minecraft. Its features were actually far more likely to be at odds with the vanilla models system than to work with it. This issue was never rectified before MCPatcher stopped updating.
I'd suggest looking into Optifine, but if any version of Minecraft old enough to be compatible with MCPatcher won't have the updated Optifine features anyway. So that's a wash as well, I'm a afraid. You'd simply need to update to a newer Minecraft version in order for this to be possible.
You're free to ask about that in the Optifine thread, but as I stated previously I'm pretty sure that it can't be done. Those features just weren't implemented yet. Of course, I may be wrong hence why you should inquire in that mod's thread.
