I was thinking this might be possible to do by editing blockstates, similar to texture variants, but there are no files for any paintings in the game's blockstates folder. Are the available paintings and which texture files they use simply hard-coded?

Yup. Paintings are actually entities, not blocks. So everything about them is hard-coded into the game's files. As such it sadly isn't possible to add new paintings without modding.

You can make map art.
Create a map, change the 64x64 area of that map to your liking and you got a 1 map art.
You can expand the canvas over multiple maps, that you can then place into item frames.
