Hello, I've heard that Optifine has a feature that changes item textures in-game when it is named something specific. I cannot find any recent tutorials on how to do this, so can someone help/walk me through?

Basically you'll create a .properties file located in YourResourcePack/assets/minecraft/optifine/cit
For example in order to change a diamond axe you would write a .properties file similar to
type=item items=minecraft:diamond_axe texture=diamond_battleaxe nbt.display.Name=battleaxe
You would then have your desired texture as .png in the same folder with the name diamond_battleaxe.png
How do i do this with models
same as above, but include a new line
model=<model name>
