I want to have a model for when there is an item in it and when there is not. I know you can do it with optifine because people have done it with jukeboxes and furnaces with specific items in them. I just don't know how to do it with item frames.

I'm pretty sure that this isn't possible since item frames, unlike jukeboxes and furnaces, are rendered as entities and not blocks.
And if I'm wrong about that... AWESOME! I'd love to be able to do this exact thing too!
But sadly, nothing I've seen has led me to believe it's possible right now. So sorry to be the bearer of bad news.

Yeah, I know. Minecraft is full of odd cases like how chests are blocks but rendered as entities. Item Frames, inversely, use a block texture but are actually entities in the world. (You can pop open MCEdit to verify this fact) It's all very confusing, really.
Mojang has either never heard the word "consistency", or has heard it and just doesn't think it's that great of a concept.
