That second one can only be gotten if you already have that achievement. Max stack size is 64. This would be impossible to obtain without a mod that increases the stack size, in which case the extra space would be useless, since you're already using a most that does more then that. As for the first one, I like it.
I like the achievements but the adaptations seem so overpowered. I mean, no fall damage?... no fire damage?... no drowning? That seems kinda rediculous. I say keep those achievements but take out the final adaptation and then move the other 2 down 1. If thats too confusing, here is and example:

I have a sugestion. I think that only a limited number of achievements should be available to you when you start but with every 1 teir you complete, you can unlock a teir of a harder difficulty or something like that.

Don't use armor or tools against mobs- Just your hands.
Kill 1000 hostile mobs at night in the 50 days,
kill 20 ghasts with bare hands.
Perks- Take 1/2 damage and deal dimond sword damage w/ bare hands.
Right click w/ bare hands= charge. Or something.

I suggest not having anything repetitive. Things like "Stand still for 1 minute" are boring and easy. And how many people fall in the void once, let alone 50 times? That's also boring. It just turns into a stream of suicides. Jumping for 1 minutes is the same as standing still, except holding down space the whole time. And staying in lava for 1 minute? That can only happen with lots of food in peaceful. I reckon my suggestions are best, though they don't have rewards. The best achievements I see are for random things with references to other games and things. Humor beats repetition, any day.

-Create a chain reaction of 20+ TNT. Reward: ADAPT: "Blood of Obsidian." (50% less damage from Explosions.)
I AM the Anti-Grief.
-Defuse 10 Creepers. Reward: None.
-Put out 50 Fires. Reward: Block. (Or the like. Infinite water kind of makes sense.)
-Detonate a block of TNT, and have it destroy no bricks. (Incase in Obsidian.) Reward: ADAPT: "Anti-Ssstanley". (Ability to "de-activate" creepers by right-clicking them. Will not explode.)
Burning Man.
-Be set on fire 5 times. Reward: None.
-Swim through lava (1+ blocks.). Reward: Flint and Steel.
-Be on fire for 20 seconds. (This will require food, and probably re-lighting.) Reward: ADAPT: "Ring(finger) of Fire". (Ability to take reduced damage from being on fire, double burn time, and light mobs on fire by touching them while on fire.)
Local Badass
-Jump off something high, and take 9 damage. And survive. Reward: Iron Boots.
-Kill a with another . Reward: Mob Spawner.
-Kill 10 skeletons in 2 minutes. Or something like that. Reward: ADAPT: "Proven Badass". (10% reduced damage from all sources, immunity to slimes, increased reach with weapons.
And I think that's enough suggestions. Now, for some mod-related suggestions. First off, everyone's favorite mod that involves Intelligent AI...
Army of Two
-Spend 5 Minecraft days with Charlotte. Reward: None.
-Heal Charlotte 10 hearts in total. Reward: Two stacks of Flowers. One stack of each.
-Travel with Charlotte to placed far and wide, and kill the inhabitants. (1+ Kill of every hostile non-aether mob.) Reward: ADAPT: "Double Trouble." (When with Charlotte, both of you recieve 1.2x damage output, reduced damage from mobs, and Charlotte cannot die. Merely be incapacitated, requiring a daring rescue.)
(Optional) -Rescue Charlotte in a Daring Fashion. ADAPT: "Prince Charming". (Doubled/Tripled healing to/from Charlotte.)
And now, the most popular Dolphin-Related mod ever, (This could be sub-divided into several different sections, perhaps "Aquestrian, Aquatics, Ogres and Wraiths, Other."

-Tame a Horse. Reward: A saddle and an Apple.
-Feed a seed to every type of Bird. Reward: A stack of Seeds. Feathers.

-Have one of every color of Dolphin tamed at once. Reward: ADAPT: "Thar' she Blows!". (Dolphins can now Dive, climb Waterfalls, attack on command, swim faster, give you limitless oxygen underwater, and maybe leap out of the water. (Flying Dolphin Race anyone?))
-Fill a Black Pegasus' chest with Armor/Tools. Reward: ADAPT: "Jouster". (Horses are immune to damage when riding them, increased rider defence.)
-Complete all the above Achievements, then Wear a Rabbit as a Hat. Reward: ADAPT: "Familiarist". (Animals will no longer attack/be afraid of you. Ogres and Wraiths are exempt. Werewolves will now fight for you. Right click while holding a golden sword to summon an army of Lions/Foxes/Birds/Dolphins?/Bunnies/Polar Bears/Sheep to fight for you.)
Whew that was a long one. What next? Oh, Colony mode!
Colonialist.
-Place one of every kind of Colony Mode Chest. Reward: Gold/Iron Scepters.
-Have your delivery boy collect 10 coal and 1 iron. Reward: None.

Army of Two
-Spend 5 Minecraft days with Charlotte. Reward: None.
-Heal Charlotte 10 hearts in total. Reward: Two stacks of Flowers. One stack of each.
-Travel with Charlotte to placed far and wide, and kill the inhabitants. (1+ Kill of every hostile non-aether mob.) Reward: ADAPT: "Double Trouble." (When with Charlotte, both of you recieve 1.2x damage output, reduced damage from mobs, and Charlotte cannot die. Merely be incapacitated, requiring a daring rescue.)
(Optional) -Rescue Charlotte in a Daring Fashion. ADAPT: "Prince Charming". (Doubled/Tripled healing to/from Charlotte.)
And now, the most popular Dolphin-Related mod ever, (This could be sub-divided into several different sections, perhaps "Aquestrian, Aquatics, Ogres and Wraiths, Other."
Familiarist
-Tame a Horse. Reward: A saddle and an Apple.
-Feed a seed to every type of Bird. Reward: A stack of Seeds. Feathers.
-Mate two Horses. Reward: A saddle and an Apple for the new horse.
-Raise a Shark Egg to full size. Reward: ADAPT: "NOT A SEAL". (Sharks will not attack you, will retaliate.)
-Tame a Green+ Dolphin. Reward: Cooked Fish.
-Tame a Pegasus. Reward: Something Epic.
-Kill 5 Lions. Reward: ADAPT: "Lion King". (Lions will not attack you, follow you sometimes, aid you in combat.)
-Kill a Werewolf in full form. Reward: Silver Sword.
-Kill a Fire Ogre. Reward: More Flames, because you don't need Hax.
-Kill 10 Wraiths in 5 minutes. Reward: Shrooms.
-Have one of every kind of Horse tamed at once. Reward: Apples, soo many Apples.
-Have one of every color of Dolphin tamed at once. Reward: ADAPT: "Thar' she Blows!". (Dolphins can now Dive, climb Waterfalls, attack on command, swim faster, give you limitless oxygen underwater, and maybe leap out of the water. (Flying Dolphin Race anyone?))
-Fill a Black Pegasus' chest with Armor/Tools. Reward: ADAPT: "Jouster". (Horses are immune to damage when riding them, increased rider defence.)
-Complete all the above Achievements, then Wear a Rabbit as a Hat. Reward: ADAPT: "Familiarist". (Animals will no longer attack/be afraid of you. Ogres and Wraiths are exempt. Werewolves will now fight for you. Right click while holding a golden sword to summon an army of Lions/Foxes/Birds/Dolphins?/Bunnies/Polar Bears/Sheep to fight for you.)
Whew that was a long one. What next? Oh, Colony mode!
Colonialist.
-Place one of every kind of Colony Mode Chest. Reward: Gold/Iron Scepters.
-Have your delivery boy collect 10 coal and 1 iron. Reward: None.
-Build 10 Colonists in total. Reward: Wool.
-Have every Colonist working with a Diamond tool at once. Reward: ADAPT: "Pioneer". (Increased run speed, increased jump height. Ability to "climb" with right-click.)
And what next? Oh, Humans+!
Man among NPCs.
-Kill 5 humans of any sort. Reward: Bones.
-Wear a full set of chainmail taken from corpses. Reward: Iron sword.
-Achieve max. Good/Evil alignment. Reward: ADAPT: "Crusader". (Increased dmg versus opposite alignment foes.
-Work with a Assassin(s) to kill either 5 Skeletons, or HeroBrine. Reward: ADAPT: "Assassin". (Increased overall damage, defence. Less gravity on arrows. Ability to craft Assassin Blades. Greater speed/jump height, better Assassin team-members.

Exterior Decorator.
-Get a Cocao fruit. Reward: Choclate bar.
-Harvest an Apple from a fruit tree. Reward: Apple Seedlings.
-Place a Wall Clock. Reward: Watch. (Irony.)
-Collect 1+ of every sort of Coral. Reward: ADAPT: "Aquarius". (Increased movement underwater, easier breaking coral, Double harvest from coral, decreased oxygen expenditure.)
-Find a Seqioa. Or however it's spelled. Reward: Ladders, an Axe.
-Plant one of every new type of Seedling, and grow them to maturity. Reward: More special saplings.
-Place 50 Topiary blocks. Reward: More topiary blocks.
-Dive to depth -50 using the diving helmet and tank. Reward: ADAPT: "Mariner". (Increased armor from Diving equipment, indefinite oxygen supply from the tank.)
