Thanks for the report, I've fixed the bug (there was a tiny typo in a single function), but luckily I knew where to look.
Rather then making a new version (1.3.4, since it was seriously a single bit of changed bytecode), I just did a silent update to V1.3.3 (if you download it again you'll get the fix). Sorry for this silly bug, and thanks again for finding it!

It seems they are looking at a block that's not supposed to be able to exist, and my WAILA block lookup code is getting upset about it. Specifically a 14th collapsing slab type (but there are only 11). It could have been created by another mod somehow, but Torch Lever's shouldn't be able to create it. If they logged out while looking at the block they will crash before they could destroy it so there are three options I could see to fix it:
Probably the easiest is that they remove WAILA from their mods folder, destroy the block (hopefully that won't cause any other issues), and then reinstall WAILA.
Second is that they log into a single player world press the WAILA config key (defaults to numpad 0), click Torch Levers, and then disable Collapsing Slabs, to disable renaming collapsing slabs in WAILA.
And third you could delete the block in some world editing software. You'll have to look for the collapsing slab block ID, there are two you need the one from config called "fallingBlock2" it defaults to 516. The offending block will have a metadata value of 5 (or 13, but probably 5). The user experiencing the crash is probably looking at the block so that could help to find it too.
I could prevent the crash directly with some changes to the code, but I suspect there would be more problems since the block shouldn't exist in the first place. Sorry about this, and I hope one of these methods can help out.

I saw your PM that this was fixed, but for anyone else experiencing issues, I believe this may have been from trying to run the mod on 1.6.1 not 1.6.4 (I didn't test the newest Torch Levers version on any older Minecraft versions).
Also I'm actively working on a Minecraft 1.7 version of this mod now, but it's still going pretty slow. I'm going to try to have it out as soon as possible, but it will take some time... I'm going to try to make your waiting worth it though so, and I will try to post some progress updates to twitter (@hitchh1k3r)...

I think you should add iron trapdoors, which are like iron doors but trapdoors with all their carpet counterparts. Also, make the spikes ether wood, stone, iron, or diamond, with potion effects and automation as modifiers you can add on, so the strongest would be automatic diamond poisoning withering slowing blinding and the weakest would be plain wood.

A lot of this was already planed for Torch Levers 2.
Just yesterday I was talking about how good iron trapdoor carpets would be, but since Iron Trapdoors are a planned feature for Minecraft 1.8 I might wait until then to add iron carpet trapdoors... I'm undecided.
The spike trap modifiers will work the way you described, but I hadn't considered tiered traps (wood, stone, iron, diamond...) and that's a great idea so I'll definitely be playing around with it.
If I make a server, can I change the options so normal members may use basic iron made spike traps and donators get access to the poison, slow, burn, wither traps?
This mod doesn't have any features like that, and I'm not aware of any other ones that do. Maybe someone else knows of something and can help you out.

A 1.7.2 update is coming, but not soon I'm working as quickly as I can but it's going to be a complete rewrite and I have a lot of new features to add.

Hey Thanks for making this mod. I am using version 1.3.2 on a FTB Monster server I am hosting. I have had my sever crash because of the spike trap issue that is fixed in 1.3.3. Can I use this updated version and rename it 1.3.2? Otherwise I am not sure if players could connect unless they update their client version of the mod as well.

There should be at least a few ways to manually update the mod on your server, depending on your setup you might just be able to delete V1.3.2 (in the server's "mods" folder) and put V1.3.3 in, no renaming necessary. I'm not sure if a V1.3.3 server will cause problems with V1.3.2 clients... try it out if you want and let us know.

That could be a problem, and should probably be fix. You'll want to report it here (http://www.minecraftforum.net/topic/1927847-/) though. That's the mod that adds the keypad.
