Two things to signal:
First the mob in the portrait sometime vanish
(I tried to optifine thing to see if it was that, I confirm that with optifine bug and without don't.)
And second: Whenever a mob is affected by something (potion or spawned effect like in Hypixel maps) I got little numbers popping on the picture of the effect, hiding it and so I can't tell which one it is...
I think those numbers are the time range of the effect... Only before it appeared under the icon, not onto it...
I'm using the new version of your mod and it works really well. The only problem I've had is that it makes my Minecraft game freeze when I use it with the Biomes O Plenty mod. The game will load and it will work if I don't play on a file with Biomes O Plenty in use, but I cannot seem to use them together. My forge is updated to the latest version. Could you please try to make these compatible?

I get this same message everytime I log into the game. But it still loads and all seems fine, are you sure this mod is doing it? Ithought it was something else but never really looked more into it, like I said the game still loads but I get that exact message everytime I play.

Its client only and works on any server now. New changes in 1.6.1 made this possible. However, for balancing, invisible players cannot be targeted by the indicator and Potion effects are not visible on servers.

There was a conflict with Bungeecord (i think that was what it was called lol) and this mod where it would detect this mod and label it as a cheat. Rich already added balancing to the server side part of the mod, but Bungeecord needs to update to change the conflict.
Fixed Mobs disappearing in portraits when using optifine.
Fixed a possible crash when clicking around in the advanced menu.
Removed almost all attribute pushing when rendering, what this means to you is better performance on slower video cards.
Fixed particles spawning out of control when a window that pauses the game is open.
Gui rendering moved to forges new RenderInGameGuiEvent, so other mods that need to hide all of the ingamegui will also hide the portrait if needed.
Donator particles a little more appearant.
Particle Rendering moved back to the tesselator as the optifine bug was fixed.

This is now fixed, turns out it was obfuscation problems or missing methods in Optifine's version of the OpenGLHelper.
Edit:
Hotfix 2.8.5.1 added that fixes a layering problem with the portrait window(mobs always rendering on top).
I am using MC 1.6.2/Forge 789.
When I add Optifine A4 I never see the mob in the Damage Indicators window.
When I do not use Optifine, I always see the mob.
