you're on 1.4.2 using the 1.4.4 version of the mod
do you have forge installed correctly?
logs or it didnt happen
are you sure you're using the 1.4.2 version of the mod instead of the 1.4.4 version? also, i used to dual boot and i found that you cant copypasta a .minecraft from one to another. maybe that's it?
I was using 1.4.2, and had downloaded it via the minecraft.exe file from minecraft.net. I checked in Magic Launcher, which tells me version info about which jar I've told it to use.
I feel like I'm going blind, but I do not see the download link anywhere in the 1st post. Did it get removed on purpose or perhaps on accident while updating?

Hey, I'm having a strange problem across both 1.4.2 and 1.4.4 versions. Using this mod at the same time as any Optifine version causes all mobs to have "garbage" textures or none at all on some limbs/the main body (garbage meaning textures from items.png/blocks.png slapped on mobs oddly). This seems to happen regardless of what settings I change in Optifine's options, and makes this mod unusable due to sheer annoyance. Everything else functions fine, but the garbage mob textures ruin it. Worst part is that with both mods and the jar updated to 1.4.4, now the player model is experiencing the same problems. Any idea what the issue or conflict is? I'm going to experiment with it and try to find the cause.
(Note: I have tested this using only Optifine, Forge, and Portal gun, the below screenshots do have other mods enabled since it seems to make no difference past the mentioned first 3.)
Here's some screenshots to better demonstrate what I mean:
The player model glitching in version 1.4.4 of Portal gun/Optifine/Minecraft (texture pack used to show skin, since the skin doesn't show up well in default):

then your computer probably cant handle both installed. or optifine somehow failed in the newer updates...

Let me clarify: I am in fact using 1.4.4 Minecraft and accordingly updated mods. What I meant was that this happened back before 1.4.4 was released, when I tried installing the 1.4.2 version of this mod on a 1.4.2 jar that had the appropriate Optifine version already in place.
Also, for some reason, when trying to test it this time, it's suddenly stopped glitching like this (missing/corrupted mob textures). Now, it seems to all cooperate just fine for no reason at all.
Nope, it's still having issues. Three-legged cows and headless pigs ahoy.
I'm still trying to test it, and even on a supposedly non-glitchy loadup, there's still some errors in the log created by Magic Launcher. Here's an error series that always shows up when Optifine and portal gun are loaded together in any order (I can't seem to find the forge log you're talking about, sorry.):

Optifine and Portal Gun do not work correctly together in any order, I've discovered. They used to back around 1.0.0 I think it was, but now? Not so much. Could you please try to make a merged class/compatibility patch for portal gun and optifine, iChun, assuming that wouldn't be too big a hassle? I don't know which classes conflict yet, but I'm going to look into it. I'll edit this post if I find anything.
Edit: Not a single conflicting file. Odd. I'm currently testing to see if cramming the portalgun folder into optifine's zip and running the Frankensteinian monster (which I've dubbed OptiPortal) with Magic Launcher fixes the issue. It seems to work so far, I'll update with any changes.

portal gun and optifine seem to have issues on certain graphics cards. i've never run into the missing parts problem with optifine and portal installed. could you post your PC's set up?

The gibberish was from mid-run.
As for specs (basic stuff off the top of my head):
Windows 7 Home Premium OS (came with the computer, no other operating systems installed)
AMD Phenom II X4 965 CPU (4 Cores running at 3.4 GHz total)
Nvidia GeForce GT520 GPU/Graphics Card
4 GB RAM, Minecraft being allowed a full 1 GB (double the default amount)
This setup works beautifully with Minecraft by the way. On default, with Far distance, all fancy graphics, no Optifine, I get 100-120 FPS. With Optifine, a 128x texture pack, Fancy settings, and Far distance, I get 70-90 FPS. I typically use a 64x or lower pack and Normal distance for aesthetic reasons.
So, as you can see, it's not caused by a lack of power. It's also not caused by incompatibilities from what my testing shows, since combining Optifine and Portal Gun into a single zip seems to have fixed it, oddly enough.

Seems like a weird question but, how do I find my PC (I'm using a laptop btw) setup?

.minecraft\config
there should be a portalgun config in there, open it and read through it. you change the values at the bottom

Can you Show me how?
I used to get the same texture glitch with the 1.2.5 versions of portal gun and optifine. remove either one and it would go away.
It used to affect mobs first, then start on chests (vanilla or ironchests) and work its way to the player.

Edit: Not a single conflicting file. Odd. I'm currently testing to see if cramming the portalgun folder into optifine's zip and running the Frankensteinian monster (which I've dubbed OptiPortal) with Magic Launcher fixes the issue. It seems to work so far, I'll update with any changes.
is it actually crashing? or just spitting out gibberish as it runs.
.minecraft\config
there should be a portalgun config in there, open it and read through it. you change the values at the bottom
portal gun and optifine seem to have issues on certain graphics cards. i've never run into the missing parts problem with optifine and portal installed. could you post your PC's set up?
Which one do I change?
