It depends on the mod. (At least from what I've seen.) Some require you to put the .zip into the mods folder, while other Modloader-compatible mods (Not necessarily Modloader dependent. Eg. TMI) you should just put the fiels in the minecraft.jar. I'm not super sure on this, but if you follow the install instructions on the mod page, and check it's compatibility (If you're using other mods.) everything should work fine.

Sorry, I don't know what's causing that error. Have you tried using the texture patcher?
I installed the ModLoader several times even looking at tutorial videos to make sure and this happens every time I boot up Minecraft.. and yes I'm using 1.0.0.. What should I do?

As has been mentioned, it depends on the mod you want to install. Some have to be installed into the jar directly, others can just be dropped into the mods folder. Some are both.

The patcher automatically deletes the META-INF folder correctly for your OS. Once the patcher is done, you should be all set.
then how come i try to use mod loader and Audiomod , my MC game shows a black screen. Is it because they share similar class files? If so, which ones should i delete?
For any of the people having a problem with crashing, I found something that fixed my problem and it might help you.
The problem turned out to be that I needed to update to Java 7. Even though it is not on the Java specifically, they have a section leading you to download Java 7 which is..
Once there you can click on Download JDK in the Java SE 7u2 category and click the one appropriate for your computer. Alternatively, if the link doesn't work, you can go to the Java website, and click "Free Java Download", then "Looking for Java 7?", then download JDK in the Java SE 7u2 category.
Hopefully this fixes the problem some people are having! (Sorry if this was already said)

There must be a problem when you install it. Install modloader, then install audio mod. Be sure to copy over ALL the files and folders from the zip files for each mod. A lot of people forget the extra folders that sometimes need to be copied over.

That's a corrupt JAR file, either something went wrong, or the zip utility you used messed up the JAR. Use 7zip or winrar if you aren't already, and start from a clean JAR file.
It aint a problem with Equivalent Echange, that's literally just modloader when i'm trying to get down to the bottom of what's not working for me, and it's modloader apparently. I'm not even using equivalent exchange

Be sure you're starting from a clean JAR file (force update from the minecraft launcher) and be sure you're using the latest download of modloader.
