Hi there, it's a bit difficult to understand the setup based on the screenshot, but here's what I can suggest based on what I can see.
You should simply need to enter 25565 in each of those three port boxes under where you've selected Port Forwarding.
As for what looks like the linked page (set up a service on your device), It'd be easiest to choose Host for part 2. As for part 3, I believe it's asking which device it should be forwarding the traffic for. If you can see the name of your computer in there, select it from the dropdown. If not, enter your computer's local IP address. This page should only need to be used if the port forwarding page doesn't ask who it should be forwarding the traffic to/from. If the port forwarding page does ask who to allow port 25565's traffic to/from, perform the same steps as below.
Obtaining your local IP address (Windows):
1. Press Windows + R, then type cmd
2. In the command console that appears, type ipconfig and press enter
3. Note your IPv4 Address and enter this into the box on step 3 ('Or manually enter a LAN IP')
