It's actually quite easy. If you notice in the top right corner there is a faint cloud. I basically used a picture of a sunset for one of the effects. All I did was use an overlay. I combined that with a bunch of other effects, plus I messed around with the colors and such to get that look.
how did you put the creeper face in the guy so well?
All I did was cut out an image of a creeper, resized it so it looked good. Then I zoomed in and with a very small size brush, and erased the part of the creeper that didn't fit within the neckline of the shirt. Then I used a tool in photoshop called a clone tool and used it to overlap the remaining parts of the mans head that was still showing.
