If the radius of 5 is centred from the command block you shouldn't even need the first set of co-ordinates.
So... Correction! /tp @a[r=5] x y z (x,y,z being the destination)
So is this correct?

try something like "/tp @a[-667,4,-259,5] -715 9 -347"
The first time I activate it, it works, but the second time it doesn't teleport me.
How are you activating and/or post pictures of what it looks like after activated the first time.
For some reason only works with a lever in this area, but putting a stone button there doesn't work.

After hours of frustration, a simple closer positioning of a command block fixes it.
