Are we limited to "standard items"? If not then cobble generators. Outside of sky block scenarios, they're just silly since you can kill a whole bunch of birds with one stone when you find yourself in need of cobble. You can excavate that land you needed done, potentially find minerals, get more xp, discover a cave or abandoned mine if you're deep enough AND get your cobble. Plus, mining stone is a TINY bit faster. Why generate cobble?
If we ARE limited to standard blocks and items: Clay. I say this as a person who plays multiplayer almost exclusively. There's simply not enough of the stuff to be valuable. It's not renewable, non creatable, takes a ton of raw materials to make a placeable block and has limited use specifically because of its limited quantity. It's actually a pretty block and the dyes make it a very attractive idea to use clay en mass...but getting the stuff is a beast, leads to a lot of competition on the server which leads to gouged landscapes, and unecessary terrain generation.
I may define "useless" a little differently than the next though.
I believe there is a difference between the most useless and most worthless item. the most worthless item would probably be stone/cobblestone because most of the world is made of it, so it wouldn't be good for currency. Items with the most worth would be things like music discs and diamonds because they are rare.
The most useless block/item would be sponge because it has no use. Not even for decoration.

The sponge. Not only does it have no function other than decoration, it is only available through creative mode and thus takes up a block ID that could be used for other things. Honestly, who uses sponges to decorate?

If we limit our scope to survival, then Netherrack. It's weak, and it's not even good for decoration. Its only use is fireplaces or netherbrick. Honestly, I don't use the former since I prefer to build with wood, and making netherbrick requires a huge amount of coal (1 for 2 blocks).
Cauldron. It takes 7 iron to make and all it does is hold water (for 4 uses iirc). Making a 2x2 hole with water in it gives you infinite amount of water.

Actually, dirt is VERY useful once you live in a desert biome... because you'll need dirt to create a farm.
